Постачальник зобов’язаний надати Замовнику протягом 5 днів з дня підписання договору належним чином засвідчені копії:
- діючого сертифікату, виданого акредитованим органом сертифікації, яким посвідчується, що система управління безпечністю харчових продуктів відповідає ДСТУ ISO 22000:2007 (ISO 22000:2005, IDT) або ДСТУ ISO 22000:2019 (ISO 22000:2018 IDT);
- сертифікати якості;
- документ про реєстрацію потужностей або експлуатаційний висновок;
- документів, які підтверджують право власності або користування складським приміщенням, де зберігається товар; документів, які підтверджують право власності/користування на: обладнання (може бути довідка в довільній формі про наявність обладнання для зберігання продуктів харчування з переліком такого обладнання), матеріальну базу та автотранспорт (для автотранспорту може бути надано копію договору на виконання /надання послуг із перевезення/транспортування відповідним транспортним засобом, що буде використовуватися при виконанні умов цього договору);
- особової медичної книжки водія транспортного засобу. Строк дії обов’язкового медичного огляду повинен бути дійсним відповідно до положень чинного законодавства;
- діючого сертифікату, виданого акредитованим органом сертифікації, яким посвідчується, що система управління якістю учасника відповідає ДСТУ ISO 9001:2015 (ISO 9001:2015).
