入 札 公 告 ( 工 事 )次のとおり一般競争入札に付します。
令和８年４月９日独立行政法人労働者健康安全機構横 浜 労 災 病 院契約担当役院長 三上 容司１ 工事概要(１)工 事 名 看護学校北側職員駐車場舗装工事(２)工事場所 神奈川県横浜市港北区小机町３２１１番地(３)工事内容 詳細は別添仕様書のとおり(４)工 期 契約締結日から令和８年６月３０日まで２ 競争参加資格(１)予算決算及び会計令(以下「予決令」という。)第７０条の規定に該当しない者であること。
ただし、未成年者、被保佐人または被補助人であって、契約締結のために必要な同意を得ている者については、この限りでない。
(２)予決令第７１条の規定に該当しない者であること。
(３)会社更生法に基づき、更生手続開始の申立がなされている者又は民事再生法に基づき再生手続開始の申立てがなされている者でないこと。
(４)厚生労働省競争参加資格において、関東・甲信越ブロックの「舗装」の競争参加資格を有する者であること。
(５)経営規模等評価結果通知書にて、「舗装」の評価を受けている者であること。
(６)主任技術者を専任配置できること。
また、当該技士は元請業者が直接雇用する者であること。
(７)その他、契約担当役が入札説明書にて定める資格を有する者であること。
３ 入札手続等(１)担当部課〒２２２－００３６ 神奈川県横浜市港北区小机町３２１１番地横浜労災病院会計課契約係 竹内電話 ０４５－４７４－８１１１(２)入札説明書の交付期間、場所及び方法公告公示日から令和８年４月２３日(木)まで上記３(１)の場所にて交付する。
(３)質問書について仕様内容等に質問がある場合は、入札説明書に添付の質問書様式により、質問を行なうこと。
質問書の受付は令和８年４月１６日(木)まで上記３(１)の場所で行う。
質問の回答は順次行い、閲覧に供する。
(４)入札書の受領期限、場所及び提出方法令和８年４月２３日(木)１２時００分まで横浜労災病院会計課契約係へ持参または郵送すること。
(５)開札の日時及び場所令和８年４月２３日(木)１５時３０分 横浜労災病院管理棟３階会議室４ その他(１)手続において使用する言語及び通貨 日本語及び日本国通貨に限る。
(２)入札保証金及び契約保証金 入札保証金は免除。
契約保証金は有。
(３)入札の無効 本公告に示した競争参加資格のない者のした入札、申請書または資料に虚偽の記載をした者のした入札及び入札に関する条件に違反した入札は無効とする。
(４)落札者の決定方法 会計細則第42条の規定に基づいて作成された予定価格の制限の範囲内で最低の価格をもって有効な入札を行った者を落札者とする。
ただし、落札者となるべきものの入札価格によっては、その者により当該契約の内容に適合した履行がなされないおそれがあると認められるときまたはその者と契約を締結することが厚生な取引の秩序を乱すこととなるおそれがあって著しく不適当であると認められるときは、予定価格の制限の範囲内の価格をもって入札した他の者のうち、最低の価格をもって入札したものを落札者とする。
(５)契約書作成の要否 要。
(６)当該工事に直接関連する他の工事の請負契約を当該工事の請負契約の相手方との随意契約により締結する予定の有無 無。
(７)関連情報を入手するための照会窓口 上記３(１) に同じ。
(８)詳細は入札説明書による。
５ 資料の作成に関する事項資料は、入札説明書に示す作成要領及び留意事項等に基づき作成するものとする。
