1. Matters for Competition
(1) Project Name: Dietary Therapy Outsourcing Contract
(2) Specifications of procured goods: As per the tender explanation document, etc.
(3) Period: April 1, Reiwa 9 - March 31, Reiwa 12
(4) Place of performance: Location designated by the hospital's contract manager
(5) Bidding method: The successful bid will be determined by comprehensive evaluation. Therefore, ① Submit documents related to technology, etc. for comprehensive evaluation. ② For the bid amount, please enter and submit the total management costs and material costs for 3 years, including various expenses.
When determining the successful bid, the bid price will be the amount obtained by adding 10% of the amount written on the bid form to the amount (if there are fractions of less than 1 yen, such fractions will be rounded down). Therefore, bidders must write the amount equivalent to 100/100 of their estimated contract amount (unit price per meal) on the bid form, regardless of whether they are taxable or non-taxable for consumption tax.
(6) Bid bond and contract deposit: Exempt.
2. Eligibility for Competition Participation
(1) Persons falling under any of the following shall not be eligible to participate in the competition.
① Persons who are not competent to conclude the contract and bankrupt persons who have not regained their rights.
② Persons who fall under any of the following items and for whom a certain period has not elapsed since the fact occurred (the same applies to those who use them as agents, sub-agents, managers, or other employees).
A) Persons who intentionally performed construction or manufacturing crudely in the performance of the contract, or who committed fraudulent acts regarding the quality or quantity of goods.
B) Persons who obstructed the fair execution of competition, or who conspired to harm fair prices or obtain unjust profits.
C) Persons who obstructed the successful bidder from concluding the contract or the contractor from performing the contract.
D) Persons who obstructed public officials from performing their duties during supervision or inspection.
E) Persons who failed to perform the contract without justifiable reason.
F) Persons who have used as agents, sub-agents, managers, or other employees in the performance of the contract within 2 years after the occurrence of any of the facts in the preceding items.
G) Persons who have committed acts similar to those in the preceding items.
H) If a person other than the eligible participant (branch office, business office, etc.) participates in the bidding, the bid will be invalid unless a letter of authorization from the eligible participant is submitted (except when submitted).
(2) Persons falling under any of the following may not be allowed to participate in the competition.
① Persons who have made false statements in the qualification application form or attached documents.
② Persons whose management status or creditworthiness has extremely deteriorated.
③ Persons who cannot respond to sudden calls.
(3) Persons registered in grades A, B, or C in "Services" or "Other" in the Kanto-Koshinetsu region of the Unified Qualifications for All Ministries. However, participation is not possible during the period of qualification suspension.
(4) Must have secured alternative guarantee in preparation for unforeseen circumstances.
(5) Other persons who meet the conditions specified in the specifications.
