We will conduct an open competitive bid as follows. Reiwa 8, July 28 Chiho Ishida, Director of Iou Hospital, National Hospital Organization, Independent Administrative Institution 1. Matters for Competition (1) Item Name: Contract for Purchase of Patient Clothing. Items are as per the attached "Itemized Breakdown". (2) Contract Period: From October 1, Reiwa 8 to September 30, Reiwa 9. (3) Delivery Location: 73-1 Iwadecho, Kanazawa City, Ishikawa Prefecture, Iou Hospital, National Hospital Organization, Independent Administrative Institution. (4) Bidding Method: ① For the bid amount, after incorporating all costs required for delivery, write the total amount obtained by multiplying the unit price per item and the planned quantity in the attached "Bid Breakdown". ② Bidders shall write the amount equivalent to 110/100 of their estimated contract amount in the bid form, regardless of whether they are taxable or tax-exempt businesses for consumption tax. ③ The first negotiation right holder will be determined as the person who bid within the limit of the total estimated price. If there are multiple such persons, they will be assigned negotiation order based on their bid price, and the first negotiation right holder will be determined. (5) Contract Method: Unit price contract based on Article 22 of the "Contract Handling Regulations of the National Hospital Organization, Independent Administrative Institution" (hereinafter referred to as "Contract Regulations"). 2. Qualifications Required for Competition Participants: (1) Must not fall under the provisions of Article 5 of the Contract Regulations. However, minors, persons under guardianship, or persons under assistance who have obtained the necessary consent for contract conclusion are considered to fall under the case of special reasons in the same article. (2) Must not fall under the provisions of Article 6 of the Contract Regulations. (3) Must possess the qualifications determined by the accounting officer based on the provisions of Article 4 of the Contract Regulations. The qualifications determined by the accounting officer are those graded as B, C, or D in "Sales of Goods" in the Ministry of Health, Labour and Welfare competitive participation qualifications (all-ministry unified qualifications) and have competitive participation qualifications in the Tokai-Hokuriku region. 3. Other Necessary Matters: (1) Language and currency to be used for bidding and contract procedures: Japanese and Japanese currency. (2) Bid bond and contract deposit: Exempt. (3) Matters required of bidders: Those who wish to participate in this open competition must submit a sealed bid form with proof of "2. (3)" attached by the bid form submission deadline. Bidders must respond to requests for explanation of the above proof from the accounting officer by the bid opening date. (4) Invalid bids: Bids submitted by persons who do not have the competitive participation qualifications indicated in this announcement, or bids submitted by persons who have not fulfilled the required business operations will be invalidated. (5) Requirement for contract creation: Required. (6) Method of determining the contract party: The negotiation right holder will be the bidder who submitted a valid bid within the limit of the estimated price created based on Article 21 of the Contract Regulations. If there are multiple such bidders, they will be assigned negotiation order based on their bid price, and the bidder who submitted the lowest bid will be the first negotiation right holder. After the first negotiation right holder is determined, negotiations will be held immediately to determine the contract price. However, if negotiations fail or if a contract is not concluded within 10 days from the start of negotiations, the accounting officer may negotiate with other negotiation right holders in order of negotiation rank.
