千葉市病院局公告第２８号一般競争入札について次のとおり公告します。
令和８年４月９日千葉市病院事業管理者 山 本 恭 平１ 一般競争入札に付する事項(１)委託件名千葉市立新病院開院記念式典等業務委託(２)委託案件の仕様等仕様書等による。
(３)履行期間契約締結の翌日から令和８年１０月３０日(金)まで(４)履行場所千葉市立幕張海浜病院 他２ 一般競争入札参加資格一般競争入札に参加を希望する者は、次のすべての要件を満たしていなければならない。
(１)令和８・９年度千葉市委託入札参加資格の審査を受け、資格を有すると認められている者であること。
(２)地方自治法施行令(昭和２２年政令第１６号)第１６７条の４の規定に該当しない者で次のいずれにも該当しない者であること。
ア 手形交換所による取引停止処分を受けてから２年間を経過しない者イ 当該入札日前６か月以内に不渡手形又は不渡小切手を出した者ウ 会社更生法(平成１４年法律第１５４号)の更生手続開始の申立てをした者で、同法に基づく裁判所からの更生手続開始決定がされていない者エ 民事再生法(平成１１年法律第２２５号)の再生手続開始の申立てをした者で、同法に基づく裁判所からの再生計画認可決定がなされていない者オ 千葉市内において、都市計画法(昭和４３年法律第１００号)に違反している者カ 千葉市物品等入札参加資格者指名停止措置要領に基づく指名停止措置等を入札参加資格申請期限の日から入札日までの間に受けている者(３)千葉市内に本店、支店又は営業所等を有する者(４)令和３年度から令和７年度までに、同種・同規模の式典等(官民問わず)の企画・運営業務を履行した実績を有すること(５)一般財団法人日本情報経済社会推進協会が認定するプライバシーマークを取得していること３ 契約事務担当課〒２６０－８７２２千葉市中央区千葉港１番１号 千葉市役所本庁舎６階千葉市病院局経営企画課総務班電話 ０４３－２４５－５７４９４ 入札参加資格確認申請書の配布及び提出一般競争入札に参加を希望する者は、入札参加資格確認申請書等を提出し、入札参加資格の確認を受けなければならない。
(１)申請書等の配布 千葉市「入札情報等」ポータルページで、「発注情報一覧」内「業務委託」のリンク(https://www.city.chiba.jp/business/hatchu/nyusatsujoho/index.html)当事業の箇所からダウンロードすること。
(２)提出場所等 公告の日から令和８年４月１５日(水)午後４時３０分(必着)までに持参又は郵便により提出すること。
持参による場合は、日曜日、土曜日及び休日を除く午前９時３０分から午後４時３０分までとする。
(３)入札参加資格の確認通知令和８年４月２０日(月)までに「入札参加資格確認結果通知書」を発送する。
５ 入札説明書等の交付前記４(１)同様、千葉市「入札情報等」ポータルページで「発注情報一覧」内「業務委託」のリンク当事業の箇所からダウンロードすること。
なお、図面については、公告の日から令和８年４月１５日(水)まで、前記３の契約事務担当課において無償により交付する(日曜日、土曜日及び休日を除く午前９時３０分から午後４時３０分まで。)６ 入札手続等(１)入札及び開札の日時 令和８年４月２４日(金)午前１０時００分(郵送の場合は、令和８年４月２３日(木)午後５時００分までに前記３の契約事務担当課へ書留郵便にて必着のこと。
)(２)入札方法 総価で行う。
(３)入札保証金 免除(ただし、千葉市病院局契約規程(平成２３年千葉市病院局規程第２５号)の規定によりその例によることとされる千葉市契約規則(昭和４０年千葉市規則第３号)第８条第２項の規定に該当する場合は、当該入札保証金の納付の免除に係る部分に相当する額を違約金として徴収するものとする。
)(４)落札者の決定方法 千葉市病院局契約規程の規定によりその例によることとされる千葉市契約規則第１０条の規定に基づいて作成された予定価格の制限の範囲内の価格で入札をした者のうち、最低の価格をもって有効な入札を行った者を落札者とする。
なお、入札金額が最低制限価格に満たない場合は、失格とする。
落札候補者となるべき同価の入札を行った者が２者以上あるときは、くじにより落札候補者を決定することとする。
(５)無効となる入札 千葉市病院局契約規程の規定によりその例によることとされる千葉市契約規則第１６条の規定に該当する入札７ その他(１)契約保証金 要(ただし、千葉市病院局契約規程の規定によりその例によることとされる千葉市契約規則第２９条に該当する場合は、免除とする。)(２)契約書作成の要否 要(３)契約手続において使用する言語及び通貨は、日本語及び日本国通貨に限る。
