Regarding the implementation of a public tender for the purchase of operating room bedside monitors
We will conduct a conditional public tender as follows.

Bidders shall comply with the following matters in addition to the relevant laws and regulations.
In this case, if there is any doubt about the specifications, you may request an explanation from the responsible department.
However, after the tender, no objection can be raised on the grounds of ignorance or uncertainty about the specifications.
Details
1. Matters subject to conditional public tender
(1) Goods and quantity: Bedside monitor, complete set
(2) Performance of goods, etc.: As per attached "Operating Room Bedside Monitor Set Specifications"
(3) Delivery deadline: November 30, Reiwa 8
(4) Delivery location: Miyazaki City Tano Hospital, 3rd Floor Operating Room (6-2, Minamihara 1-chome, Tano-cho, Miyazaki City)
(5) Tender method: A tender will be held for the above goods.
The successful bid price will be the amount obtained by adding 10% of the amount written on the bid form to that amount (if there is a fraction of less than 1 yen, the fraction will be rounded down).
Therefore, bidders shall enter the amount equivalent to 110/100 of the estimated contract amount into the Miyazaki City Goods Electronic Tender System, regardless of whether they are taxable or tax-exempt businesses for consumption tax and local consumption tax.
2. Special contract provisions
(1) Miyazaki City shall terminate the contract in any of the following cases during the contract period for this tender.
① If the contractor violates the contract terms.
② If the contractor is found to have close ties with a violent group (a violent group as defined in Article 2, Item 2 of the Act on the Prevention of Unjust Acts by Violent Group Members (Act No. 77 of 1991)) or a violent group member (a violent group member as defined in Item 6 of the same Article).
(2) Miyazaki City shall not be liable for damages incurred by the contractor due to contract termination.

3. Bidder qualifications
Bidders must meet all of the following requirements.
① Must be listed on the Miyazaki City Designated Competitive Bidder List and have "Medical and Scientific Equipment" as their primary or secondary business type.
② Must have their head office or branch office (including business office) within Miyazaki City.
③ Must be able to reliably deliver the goods and quantities to be delivered.
④ Must be recognized as being able to meet the performance of the goods to be delivered and reliably set up the goods.
⑤ Must not fall under the provisions of Article 167, Paragraph 4, Item 1 of the Local Autonomy Act Enforcement Regulations.
⑥ Must not have been subject to a suspension of bidding eligibility under the Miyazaki City Bidding Eligibility Suspension Rules (Miyazaki City Public Notice No. 368, April 21, Reiwa 7) on any day from the date of this public notice to the date of confirmation of bidding eligibility.
4. Submission of documents for bidding eligibility
Miyazaki City will conduct a preliminary screening through the Miyazaki City Goods Electronic Tender System to determine if bidders who have submitted their bids meet the bidding eligibility requirements.
If the eligibility requirements are not met, the bid will be invalidated, and an invalidation notice will be sent before the opening of bids.
5. Display of contract terms and delivery of specifications, etc.
The contract terms will be displayed, and the place and period for delivery of specifications, etc., will be as follows. Related documents for specifications, etc., can be downloaded from the Miyazaki City website.

6. Tender briefing session
No tender briefing session will be held.
If you have any questions regarding this tender, please submit them by email or fax by September 10, Reiwa 8 (Thursday) at 4:30 PM.

Answers to questions will be published on the Miyazaki City website by September 14, Reiwa 8 (Monday).

7. Tender and opening of bids
(1) Place and date/time of tender
① Tender deadline: September 18, Reiwa 8 (Friday), 11:00 AM
② Opening of bids: September 18, Reiwa 8 (Friday), 11:05 AM
(2) Bidders must submit their bids through the Miyazaki City Goods Electronic Tender System.
If, for unavoidable reasons, it is necessary to switch to a paper bid, please contact the Miyazaki City Contract Division the day before the tender deadline.
(3) In the case of a paper bid, qualified bidders or their proxies must be present at the opening of bids.
(4) If there is no successful bidder after the opening of bids, a re-tender will be held.
(5) If collusion or other improper conduct is recognized, making it impossible to conduct the competitive tender fairly, the opening of bids may be postponed or canceled.
8. Security deposit
A security deposit of 10% or more of the contract amount, or collateral in lieu thereof, must be paid or provided.
However, the security deposit may be waived in any of the following cases.
A. When the contractor has concluded a performance guarantee insurance contract (10% or more of the contract amount) with an insurance company, with the City as the insured.
B. When it is recognized that there is no need to pay a security deposit due to the nature or purpose of the contract, and there is no risk that the contractor will fail to perform the contract.
9. Invalid bids
The following bids will be considered invalid.
Furthermore, bidders whose bids are invalidated cannot participate in re-tenders.
(1) Bids submitted by persons who do not have bidding qualifications.
(2) Two or more bids submitted by the same person.
(3) Bids submitted by a person authorized by two or more persons.
(4) Bids that violate tender conditions.
(5) Bids involving collusion or other fraudulent acts.
10. Method of determining the successful bidder
(1) The bidder who submitted the lowest bid within the estimated price will be the successful bidder.
(2) If there are two or more bidders who submitted the same lowest bid, the successful bidder will be determined using the electronic lottery function of the Miyazaki City Goods Electronic Tender System.
