Within 1 (one) working day from the date of conclusion of the agreement, the Supplier shall provide the Customer with the following documents:
- original or supplier-certified copy of the manufacturer's/supplier's declaration (or quality certificate) of the proposed goods;
- manufacturer's guarantee letter for the proposed goods, confirming the manufacturer's cooperation with the Supplier (if the Supplier is not the manufacturer of the proposed goods);
- supplier-certified copy of the Supplier's operating permit;
- supplier-certified copy of the document confirming the registration of the Supplier's facilities used for the transportation of goods. If the Supplier uses transportation services, a document certified by the carrier confirming the registration of the carrier's facilities (by the relevant food market operator);
- supplier-certified copy of the document confirming that the Supplier has storage facilities (with freezing equipment) suitable for storing goods and specialized refrigerated vehicles (at least two units) that will be used for the supply of goods;
- supplier-certified copy of the contract for disinfection, disinsection, deratization of the Supplier's specialized refrigerated vehicles that will be used for the supply of goods, and storage facilities with freezing equipment where the goods will be stored, with institutions authorized to carry out such work, as well as acts of work performed (periodically not less than once a month) in 2026, under the provided contracts;
- supplier-certified copy of the test report issued no earlier than the second quarter of 2026 (surface swabs), confirming the results of bacteriological quality control of disinfection of storage facilities with freezing equipment suitable for storing goods and specialized refrigerated vehicles of the Supplier that will be used for the supply of goods;
- supplier-certified copy of the dosimetric control report for the Supplier's specialized refrigerated vehicles that will be used for the supply of goods, issued no earlier than the second quarter of 2026 by an institution authorized to carry out dosimetric control;
- supplier-certified copy of valid personal medical books (in accordance with Order of the Ministry of Health of Ukraine No. 150 dated 21.02.2013) with an entry confirming the admission to work of drivers (at least two) and forwarders (at least two) of the Supplier who will be involved in the execution of the Agreement;
- supplier-certified copy of the act drawn up based on the results of a planned (unplanned) state control (inspection) measure regarding the compliance by the food market operator - the Supplier, with the requirements of food legislation, issued no earlier than the second quarter of 2026, with no violations of legislative requirements detected;
- supplier-certified copy of the inspection report compiled by the territorial body of the State Service of Ukraine on Food Safety and Consumer Protection based on the results of a state control measure in the form of an audit of constantly operating procedures based on HACCP principles, issued no earlier than the second quarter of 2026 for the Supplier's facilities (storage facilities).
