Non-chemical and hematological medical supplies for single use

Dear Participants!
1. Due to the imperfect presentation of names and descriptions of medical and technical requirements for goods in Prozorro market, which are necessary for the procurement Customer, we kindly ask you to pay close attention to the Specification set out in Appendix No. 1 to the Draft Agreement.
2. By submitting a price proposal, the participant confirms that the offered goods are not of origin from the Russian Federation/Republic of Belarus/Islamic Republic of Iran, and also at the stage of concluding the contract must provide the Customer with documents specified in clause 4.8. section 4 of the Draft agreement.
3. In accordance with the provisions of Resolution of the Cabinet of Ministers of Ukraine dated October 12, 2022 No. 1178, as amended and supplemented by Resolution of the Cabinet of Ministers of Ukraine dated January 19, 2026 No. 46, the Supplier at the stage of concluding the Agreement must provide the Customer with information about the ultimate beneficial owner by providing documents specified by the Customer in clause 13.3 of Section 13 of the Draft Agreement, namely: Extract / Excerpt from the Unified State Register of Legal Entities, Individual Entrepreneurs and Public formations, as well as an information certificate compiled in an arbitrary form signed by an authorized person.
4. Terms and conditions of goods delivery: Goods are delivered in batches, within 3 (three) working days from the moment the Buyer places an order. The delivery of goods is carried out by the Supplier on DDP "Incoterms 2020" terms by the Seller's transport to the address: Kyiv, Mykola Zakrevsky Street, 81/1, 02232, Ukraine, Communal non-profit enterprise "Consultative and Diagnostic Center" of Desnyansky district.
5. Payment terms: deferred payment. Payment is made within 20 banking days after the delivery of goods to the Buyer and signing of the delivery notes for the goods by the Parties.
