In accordance with paragraph 57 of Resolution No. 822 of September 14, 2020, to confirm that the country of origin of the goods is not the Russian Federation/Republic of Belarus/Islamic Republic of Iran, the Participant whose proposal is recognized as economically advantageous by the electronic procurement system based on the evaluation results must provide a scanned copy of the original document of origin of the goods, obtained from the manufacturer or official representative, or dealer, or distributor authorized by the manufacturer to distribute its products in Ukraine, for the proposed goods, within two working days. The document must contain: the procurement identifier - the announcement number published on the official website of the Authorized Body; the full name of the Supplier; the name of the procurement item, the country of origin of the goods, and must be addressed to the Buyer. The provided certificates are an integral part of this Agreement.
Delivery of Goods is carried out by the Supplier in batches as needed according to the Buyer's request (sent by electronic, telephone, or fax communication), no later than 3 working days after receiving the request. Place of delivery (transfer) of goods: KNP "Kirovohrad Regional Hospital of the Kirovohrad Regional Council", 25030, Kropyvnytskyi, Universytetskyi Ave, 2/5. With the goods being brought to the 7th floor.
