DK 021:2015: 33140000-3 - Medical supplies (33141000-0 Non-chemical and hematological disposable medical supplies)

The term of delivery of goods is according to the Customer's requests until 31.12.2026. The supply of goods is carried out based on written or oral requests from the Customer within 5 days from the moment of their receipt. The volume of each batch is determined by the Customer depending on the actual need. A batch of Goods under the Contract is considered the supply of Goods in the volume determined for each individual Customer request. The place of delivery (transfer) of goods is carried out at the address: Zdrovya St., 1, Berdychiv, 13300. The supply of goods is carried out by transport and at the expense of the Supplier. Acceptance of Goods by quantity is carried out on the basis of accompanying documents, and by quality – in accordance with quality certificates, registration certificates, quality passports, etc. The fact of non-fulfillment and/or improper fulfillment by the Supplier of its obligations under the procurement contract is confirmed by documents, including, but not limited to: documents confirming the delay in delivery of goods and/or delivery of goods of inadequate quality, etc.
A corresponding unilateral act on non-fulfillment (improper fulfillment) by the Supplier of its obligations under the procurement contract is drawn up by at least three representatives of the Customer and is certified by their signatures. This act is sent by the Customer to the Supplier within two working days from the date of its compilation. The Supplier guarantees that the goods supplied under this Contract are not goods originating from the Russian Federation/Republic of Belarus/Islamic Republic of Iran in accordance with the Procedure for the formation and use of an electronic catalog, approved by the Resolution of the Cabinet of Ministers of Ukraine dated September 14, 2020 No. 822.
