Noise measurement services b. Traffic volume survey services c. Average driving speed measurement services d. Data input, update, and calculation processing services using a planar evaluation system (4) Contract Period: From the date of contract signing until March 31, 2025 (5) Other: The tender for this service contract will be conducted with the estimated price publicly announced. However, a minimum reserve price will not be set. Estimated Price: 1,308,000 yen (excluding consumption tax and local consumption tax) 2. Qualifications Required for Tender Participation Participants must meet all of the following requirements and have their qualifications confirmed: (1) Not falling under the provisions of Article 167-4 of the Local Autonomy Act. (2) Not having filed for commencement of rehabilitation proceedings or not having had such proceedings filed against them under Article 21, Paragraph 1 or 2 of the Civil Rehabilitation Act (Act No. 225 of 1999). However, a person for whom a decision to commence rehabilitation proceedings has been made under Article 33, Paragraph 1 of the same Act shall be deemed not to have filed for commencement of rehabilitation proceedings or not to have had such proceedings filed against them if a decision to approve the rehabilitation plan under Article 174, Paragraph 1 of the same Act has become final. (3) Not having filed for commencement of corporate reorganization proceedings or not having had such proceedings filed against them under Article 17 of the Corporate Reorganization Act (Act No. 154 of 2002). (This includes applications for commencement of reorganization proceedings under Article 30, Paragraph 1 or 2 of the former Corporate Reorganization Act (Act No. 172 of 1952; hereinafter referred to as "former Act") concerning reorganization cases that are still governed by the former Act pursuant to the provisions of Article 2 of the Supplementary Provisions of the same Act. Hereinafter referred to as "application for commencement of reorganization proceedings.") However, a person for whom a decision to commence reorganization proceedings has been made under Article 41, Paragraph 1 of the same Act (including a decision to commence reorganization proceedings under the former Act based on a former reorganization case) shall be deemed not to have filed for commencement of reorganization proceedings or not to have had such proceedings filed against them if a decision to approve the reorganization plan under Article 199, Paragraph 1 of the same Act (including a decision to approve the reorganization plan under the former Act based on a former reorganization case) has been made. (8) Able to assign a technician with the qualification of Environmental Measurer (Noise/Vibration) with proof of employment relationship to this work, and the company possesses a Measurement Certification Business Registration Certificate. (9) Have concluded and faithfully performed contracts for similar work with the national government or other local public entities with a contract amount equal to or exceeding the estimated price of this work (1,438,800 yen including tax) between April 1, 2021, and the application deadline, or with this city with a contract amount equal to or exceeding half of the estimated price of this work (719,400 yen including tax) during the same period. 3. (i) The inner envelope shall contain the tender form (Form 1), be sealed with adhesive, and the front of the envelope shall be marked with the tender subject, the bidder's trade name or name, title and name of the representative, and the warning "DO NOT OPEN". (iv) For the purpose of determining the successful bidder, the contract amount will be the amount stated in the tender form (Form 1) plus 10% of that amount (rounded down to the nearest yen if there is a fraction of less than one yen). Therefore, bidders must state in their tender form the amount equivalent to 100/110 of their estimated contract price, regardless of whether they are taxable or exempt from consumption tax and local consumption tax. 6 (v) The number of tenders for this tender is one. Tenders submitted with multiple tender forms for the same item will be invalidated. (vi) Please keep the "Sender's Copy" carefully until after the tender opening. The delivery status of the mail can be checked by phone or on the postal service website using the tracking number provided on the postal receipt. (vii) The mailed inner envelope will be stored unopened until the tender opening date and time. (viii) Costs incurred in preparing documents shall be borne by the tender participant. (ix) Submitted documents will not be returned. (4) Notification of Tender Participation Qualification Results: Tender participation qualifications will be reviewed based on the submitted application documents. The results of the qualification review will be published on the city's website (https://www.city.kadoma.osaka.jp/) only for those whose qualifications are recognized. Applicants whose qualifications are not recognized will be notified individually via FAX or email with reasons. This public announcement and notification will serve as the qualification result notification for those whose qualifications are recognized. If a bidder loses their qualification to participate before the tender opening, their bid will not be opened. c. Tender Security Exempted in accordance with Article 7, Item 3 of the Kadoma City Regulations on Contracts (1964, Regulation No. 7). 5. Withdrawal of Tender Participation Application If you wish to withdraw your tender participation application after mailing the tender documents, please submit the Application for Withdrawal of Tender Participation (Form E) in person or by mail to the tender document mailing address by the tender opening time. If mailing by the deadline is not possible, please send it by FAX or email by the tender opening time and submit the original later. 7 Note: It is not possible to change or withdraw the mailed tender form and cost breakdown without withdrawing the tender participation application. 6. Tender Opening (1) The tender opening will be conducted at the following date and time, and place, with at least one witness or a city employee not involved in the tender procedures present. a. Up to two witnesses will be selected in the order of receipt. a. After sending, please call to confirm between 9:00 AM and 5:30 PM (excluding 12:00 PM to 12:45 PM) on weekdays (excluding Sundays, Saturdays, and national holidays). b. Witness Application Submission Address: Same as 15. c. If a person other than the bidder wishes to attend, they must bring a Power of Attorney for Attendee (Form H) to the tender opening. d. Selected witnesses will generally be notified by phone. (3) Determination of Successful Bidder: a. In this tender, the successful bidder will be the bidder who submits the lowest bid within the limit of the estimated price, or, if a minimum reserve price has been set in advance in accordance with Article 167-10, Paragraph 2 of the Local Autonomy Act, the bidder who submits the lowest bid among those who bid at or above the minimum reserve price within the limit of the estimated price. b. If there are two or more bidders with the same lowest bid, the successful bidder will be determined by lottery. 8 7. Publication of Tender Results, etc. (1) The results of the successful bid determination will be notified only to the successful bidder.
