SECTION 1. Matters for Limited General Competitive Tender:
1. Contract Number: 2026002886
2. Project Name: Safe Community Citizen Awareness Survey Results Data Creation Services
3. Place of Execution: Location designated by Koriyama City.
4. Period of Performance: From the contract signing date to October 30, Reiwa 8.
5. Scope of Work: Data entry work for response results obtained from the Safe Community Citizen Awareness Survey.
6. Payment Terms: Within 30 days from the date of submission of a proper invoice after completion of work (lump sum payment).
7. Minimum Reserve Price: In this case, a minimum reserve price will be set pursuant to the provisions of Article 167-10, Paragraph 2 of the Local Autonomy Act (Reiwa 22, Ordinance No. 16; hereinafter referred to as "Enforcement Regulations").
8. Other:
(1) This project will be executed via electronic bidding. Pursuant to Article 6 of the Koriyama City Electronic Bidding Implementation Guidelines for Business Contracts, etc. (established March 24, Reiwa 2; hereinafter referred to as "Implementation Guidelines"), bidding procedures shall be conducted using the electronic bidding system in principle.
(2) Design documents, etc., can be viewed through the "Goods and Services" entrance of the Tender Information Disclosure System.
(3) Forms related to bid participation can be downloaded from the Koriyama City website.
SECTION 2. Schedule for Bidding Procedures, etc.:

SECTION 3. Bidding Method:
Bidders who meet the participation qualifications shall submit their bid forms via the electronic bidding system.

SECTION 5. Qualifications Required for Bidders:
The qualifications for participating in this tender are as follows:
1. Not falling under the provisions of Article 167-4 of the Enforcement Regulations.
2. Must have received certification based on the "Koriyama City General Competitive and Designated Competitive Tender Participant Qualification Examination Guidelines" (established September 6, Reiwa 6) and be registered in the Reiwa 7 & 8 Competitive Tender Participant Qualification List at the time of the bid participation application deadline, in the computer-related services for business contracts.
3. Not currently subject to a suspension of designation under the "Koriyama City Competitive Tender Qualification Suspension Measures Ordinance" (established March 28, Reiwa 7; hereinafter referred to as "Suspension Ordinance") (including those who have received suspension measures by the bid opening date).
4. Not a party for whom commencement of rehabilitation proceedings or reorganization proceedings has been filed under the Company Rehabilitation Act (Reiwa 14, Act No. 154) or the Civil Reorganization Act (Reiwa 11, Act No. 225). However, parties for whom commencement of rehabilitation or reorganization proceedings has been filed, but for whom termination of rehabilitation or reorganization proceedings has been decided, will be deemed as if no such filing was made.
5. Officers, etc., must not be recognized as members of a violent group as defined in Article 2, Item 2 of the Koriyama City Anti-Violence Ordinance (Reiwa 24, Koriyama City Ordinance No. 46) or as persons of social notoriety as defined in Article 8.
6. For members of a cooperative association under the Small and Medium-sized Enterprise Cooperative Act (Showa 24, Act No. 181), the affiliated cooperative association must not be participating in this tender.
7. Must have their head office located within Koriyama City.
8. Must possess a Privacy Mark.
SECTION 6. Response to Inquiries Regarding Design Documents, etc.:
1. If you have any questions regarding the design documents, etc., please submit the "Design Documents, etc., Inquiry Form" (Form No. 5) via email to the following department within the inquiry period, and report by phone to confirm receipt.

SECTION 7. Application for Bid Participation:
1. Bidders wishing to participate must carefully read the design documents, etc., and then submit the "Bid Participation Application Form" (Form No. 3) and "Bid Participation Qualification Confirmation Documents" (hereinafter referred to as "Application, etc.") to the Mayor of Koriyama City via the electronic bidding system, and obtain confirmation of their eligibility to participate in the tender for this project.

* If the total file size of the bid participation qualification confirmation documents (only one file can be attached) exceeds 3 megabytes, submit only the "Bid Participation Application Form" stating "Bid Participation Qualification Confirmation Documents to be submitted separately" via the electronic bidding system, and submit the entire set of bid participation qualification confirmation documents in person or by email to the Citizen Affairs Department, Safe Community Division within the bid participation application period.
2. Notification of Confirmation Results: The Mayor of Koriyama City will notify the applicant of the results of the bid participation qualification confirmation via the electronic bidding system.
SECTION 8. Bid Bond:
Exempt.
However, if the successful bidder fails to conclude the contract, an amount equivalent to 5% of the bid amount must be paid.
SECTION 9. Amount to be Entered on Bid Form:
For the purpose of determining the successful bidder, the bid price will be the amount entered on the bid form plus the amount of consumption tax and local consumption tax (rounded down to the nearest yen if there are fractions of less than one yen). Therefore, bidders, whether they are taxable or exempt from consumption tax and local consumption tax, shall enter the amount excluding consumption tax and local consumption tax on the bid form.
SECTION 10. Suspension, etc., of Bidding:
If it is deemed that the fair execution of the tender is hindered, the tender may be suspended, postponed, or the bidding method may be changed.
Furthermore, if system failures or other unavoidable circumstances occur with the electronic bidding system, etc., the bid opening date and time may be postponed, or the bidding method may be changed to paper bidding.
SECTION 11. Invalid Bids:
Bids submitted by parties who do not meet the qualifications required for participation in this tender, bids submitted with false applications, and bids that violate the conditions for electronic bidding will be invalid.
SECTION 12. Determination of Successful Bidder, etc.:
1. The successful bidder will be the one who submits the lowest bid price among those who bid at or above the minimum reserve price within the limit of the estimated price. However, if two or more bidders submit the same lowest bid price, the successful bidder will be determined by electronic lottery.
2. The number of bid rounds is generally limited to two. However, if no successful bidder is found even after re-bidding, a discretionary contract may be concluded in accordance with the provisions of Article 167-2, Paragraph 1, Item 8 of the Enforcement Regulations.
(The submission of estimate forms is generally limited to two rounds.) The date and time for submitting bid forms and estimate forms for re-bidding and price comparison (generally on the same day as the bid opening) will be notified via the electronic bidding system along with the notification of re-bidding.
3. The bid results will be posted on the Koriyama City website.
SECTION 13. Contract Conclusion and Contract Creation:
1. Must be concluded promptly after the successful bidder is determined.
2. This project can be concluded by electronic contract.
3. If the successful bidder wishes to conclude by electronic contract, they must submit the "Electronic Contract Consent Form and Email Address Application" to Koriyama City on the day of the bid opening.
4. The contract will be prepared by Koriyama City.
5. If the successful bidder falls under any of the following after being determined as the successful bidder and before the contract is concluded, the contract may not be concluded:
(1) No longer meets any of the requirements listed in Section 5 of this announcement.
(2) Has been subject to suspension measures under the Suspension Ordinance.
(3) Circumstances arise that make it difficult to perform the contract.
6. Koriyama City shall not be held responsible for any damages arising from the failure to conclude a contract pursuant to the provisions of item 5.

Article 167-4 of the Local Autonomy Act
Except in cases of special circumstances, local public bodies shall not allow the following persons to participate in general competitive tenders:
1. Persons who do not have the ability to conclude the contract related to the tender.
2. Persons who have been declared bankrupt and have not had their rights restored.
3. Persons falling under any of the items in Article 32, Paragraph 1 of the Act on Prevention of Unfair Acts by Members of Violent Groups, etc. (Reiwa 3, Act No. 77).
Local public bodies may, for a period not exceeding three years, prohibit persons falling under any of the following items from participating in general competitive tenders. This also applies to their agents, managers, other employees, or bidding agents.
1. When the person has intentionally performed construction, manufacturing, or other services in a shoddy manner, or has engaged in fraudulent acts regarding the quality or quantity of goods in the performance of a contract.
2. When the person has obstructed the fair execution of competitive tenders or auctions, or has colluded to impair fair price formation or obtain unjust profits.
3. When the person has obstructed a successful bidder from concluding a contract or a contractor from performing a contract.
4. When the person has obstructed the official duties of staff members during the supervision or inspection conducted pursuant to the provisions of Article 234-2, Paragraph 1 of the Local Autonomy Act.
