THE WINNING PARTICIPANT must provide the following list of documents to confirm compliance no later than one day before the contract conclusion deadline:
Certificate of availability of equipment, material and technical base, and technologies.
The certificate is supplemented by (for each specified facility):
- documents confirming ownership of the specified storage facilities, or lease agreement(s) for the facility with an acceptance-transfer certificate and an excerpt from the technical passport of the facility (documents must be valid on the date of submission of the proposal and have a validity period of at least until 31.12.2026);
- documents issued in the participant's name confirming state registration of facilities by authorized bodies (provided for storage facilities), namely a decision on registration of facilities and an operational permit;
- scanned copy of the original contract for the complex of deratization, disinfection, and disinfestation of food storage facilities, the information about which was provided according to form 1 of this appendix (documents must be valid on the date of submission of the proposal and have a validity period of at least until 31.12.2026);
- expert opinion confirming the procedure for detecting bacterial contamination by swabbing of the storage facility (bacteriological studies to detect Escherichia coli (coliforms) and Salmonella bacteria), conducted in 2026;
- documents confirming ownership of the specified vehicle(s), or lease agreement(s) for vehicles, or service agreement(s) for the transportation of food products (raw materials) (documents must be valid on the date of submission of the proposal) along with documents confirming the ownership of the lessor or carrier of the specified vehicles (documents must be valid on the date of submission of the proposal and have a validity period of at least until 31.12.2026);
- documents issued in the participant's name confirming state registration of facilities by authorized bodies, related to the supply (transportation) of food products;
- contracts for sanitary treatment of the specified vehicle(s) (valid until at least 31.12.2026);
- a valid certificate(s) of sanitary treatment for each of the specified vehicles, which must contain at least three marks of sanitary treatment, allowing to track the frequency of treatment of vehicles for the transport of food products in accordance with sanitary rules and norms, and an expert opinion confirming the procedure for detecting bacterial contamination by swabbing of the vehicle body (bacteriological studies to detect Escherichia coli (coliforms) and Salmonella bacteria), conducted in 2026;
- a protocol (issued no earlier than the second half of 2025) of measurement of ionizing radiation in the specified vehicle, confirming the compliance of background gamma radiation levels and beta radiation surface activity levels in the specified vehicle to be used for food supply.
The supplier ensures the proper sanitary maintenance of storage facilities, vehicles, equipment, and inventory used for storage and supply of food products.
To confirm that the Participant complies with food legislation and applies hazard analysis and critical control point systems (HACCP system) in its operations during the production and/or circulation of food products, taking into account the Law of Ukraine "On the basic principles and requirements for food safety and quality", must provide:
- an act drawn up based on the results of a planned (unplanned) state control (inspection) measure regarding the compliance of food business operators with food legislation, in the form approved by the order of the Ministry of Agrarian Policy and Food of Ukraine No. 1503 dated 08.08.2023, which confirms the participant's (food business operator's) compliance with the requirements of current legislation and without any non-conformities and violations of current legislation, issued by the competent authority no earlier than the second half of 2025. This act must be issued for the inspection (survey) of the facilities specified by the Participant in the certificate of availability of equipment, material and technical base, and technologies (form 1), namely the inspection (survey) of storage facilities involved in food storage, and vehicles that will be used by the participant for the transport of food products;
- provide documents confirming the implementation of the HACCP system by the participant - food business operator, taking into account the Law of Ukraine "On the basic principles and requirements for food safety and quality", namely:
a valid document confirming the presence of designated persons responsible for controlling the application of constantly operating procedures based on the principles of hazard analysis and critical control points for these procedures, during the production and circulation of food products in accordance with the Law of Ukraine "On the basic principles and requirements for food safety and quality" No. 771/97 of 23.12.1997;
a valid document confirming that the participant's employees (at least one) have undergone training on the requirements of the Law of Ukraine "On the basic principles and requirements for food safety and quality"; requirements of standards DSTU ISO 9001:2015 (DSTU EN ISO 9001:2018); DSTU ISO 14001:2015; DSTU ISO 22000:2019 (ISO 22000:2018, IDT); DSTU ISO 45001:2019 (ISO 45001:2018, IDT) and the procedure for conducting audits in accordance with DSTU ISO 19011:2019 (ISO 19011:2018, IDT), with the acquisition of practical skills to conduct internal audits in accordance with the requirements specified in...
