Regarding public competitive bidding for Nara City Staff Stress Check Services (FY Reiwa 8), in addition to the provisions of the Nara City Contract Regulations (Showa 40 Nara City Ordinance No. 43) and related laws and regulations, the bidding documents shall apply. Those who wish to participate in the bidding should read the related documents carefully and participate in the bidding.
1. Matters for Bidding
Service Name: Nara City Staff Stress Check Services (FY Reiwa 8)
Service Location: Health Management Office, 6th Floor, Central Building, Nara City Hall
Service Period: From the date of contract conclusion to March 31, Reiwa 9
Service Overview:
* Stress check survey (web)
(Includes support for those who wish to take the survey on paper)
* Group analysis and MIRROR survey report creation
* Reporting of implementation results and proposal of improvement measures
(The Education Board will also be implemented simultaneously, and a separate contract will be concluded at the unit price determined by bidding. Delivery items will also be delivered separately.)
2. Qualifications Required for Bidders
Bidders must be qualified to participate in competitive bidding for the procurement of goods and other contracts to be awarded by the city in FY Reiwa 8, or must have submitted all necessary documents for the application for qualification to bid for goods and other contracts, and must meet all of the following criteria as of the date of the announcement:
* Not falling under the provisions of Article 167-4 of the Local Autonomy Act Enforcement Order (Showa 22, Cabinet Order No. 16).
* Not delinquent in national and Nara City taxes.
* Not within the period of suspension of bidding based on the Nara City Measures for Suspension of Bidding.
* Not having filed for commencement of rehabilitation proceedings under the Corporate Reorganization Act (Reiwa 14, Act No. 154) or for commencement of reconstruction proceedings under the Civil Reconstruction Act (Heisei 11, Act No. 225) (excluding those who have received a decision to approve a rehabilitation plan under the Corporate Reorganization Act or a reconstruction plan under the Civil Reconstruction Act).
* Not a violent group as defined in Article 2, Item 2 of the Act on Prevention, etc. of Unjust Acts by Members of Violent Groups (Heisei 3, Act No. 77), an organization with officers and employees who are members of a violent group as defined in Item 6 of the same Article, or an organization that conducts activities for their benefit.
* Having at least two business consignment実績 (track records) of similar scale to this project with national or local governments in the past two years.

4. Place and Time of Bid Opening

5. Matters Regarding Bid Security
At the time of bidding, the prescribed bid security stipulated in Article 4 of the Nara City Contract Regulations must be submitted. However, this shall be exempted if it falls under Article 2, Item 2 of the same Article.
6. Bid Participation Application
Those who wish to apply for bid participation must submit the following documents:
a. (Form No. 1) General Competitive Bidding Participation Application Form
(Please be sure to attach a return envelope (with postage) for the city to mail the "Bid Participation Approval (Disapproval) Letter".)
b. (Form No. 2) Business Performance Record
c. Copy of documents certifying that P-Mark (Privacy Mark) or ISO27001 (Information Security Management System) or equivalent certification has been obtained.
Bid Participation Application Method:
Primarily by mail. Documents from 1. must be submitted to the Nara City General Policy Department, Personnel Division by registered mail or express registered mail, with the postmark valid by Wednesday, October 14, Reiwa 8. Also, write "Application for Participation in Bidding for Nara City Staff Stress Check Services (FY Reiwa 8) Enclosed" on the envelope. In case of hand-delivery, please contact us in advance and deliver directly to the submission location. Reception is from 9:00 AM to 5:00 PM (excluding 12:00 PM to 1:00 PM) on weekdays, excluding Saturdays, Sundays, and holidays.
7. Bid Participation Approval
Approval or disapproval of bid participation will be notified by Friday, October 16, Reiwa 8, to those who have applied for bid participation. The notification will be sent to the email address indicated on the "General Competitive Bidding Participation Application Form (Form No. 1)", and the original document (with official seal) will be mailed later.
8. Matters Regarding Bidding
This bid shall be conducted in accordance with the Nara City Contract Regulations and laws, as well as the provisions of these conditions.
Bidding Method: Mail-in Bidding.
Write the amount on the "Bid Form (Form No. 4)", put it in an envelope labeled as per "Mail-in Bidding Envelope (Form No. 5)", seal it, and submit it by registered mail or express registered mail.
Bid Submission Deadline: Thursday, October 22, Reiwa 8
Bid Submission Destination: Post Office within Nara City Hall
The bidder with the lowest price within the bid comparison price will be the successful bidder. Bids will be made for the total amount obtained by multiplying the unit price for each service item by the planned quantity. If there are two or more bidders with the same lowest price, the successful bidder will be determined by drawing lots by an employee unrelated to the bidding process. If there is no bid at or below the estimated price, a re-bid will be held. The contract amount will be a unit price contract.
Re-bidding is limited to one time. If a re-bid is held, notification will be given separately.
Bidders shall write in the bid form the amount equivalent to 100/110 of the estimated contract desired amount, regardless of whether they are subject to consumption tax and local consumption tax.
When determining the successful bidder, the successful bid price will be the amount obtained by adding 10/100 of the amount written in the bid form to that amount (if there is a fraction of less than 1 yen, that fraction will be rounded down).
If it is deemed inappropriate to conduct this bid due to fraudulent acts by a bidder or for other reasons, the bid may be canceled. Furthermore, even after the bid is executed, the award decision may be suspended and the bid canceled.
Bidders must not engage in acts that violate the Act on Prohibition of Private Monopolization and Maintenance of Fair Trade (Showa 22, Act No. 54), etc.
9. Attendance at Bid Opening
One bid opening observer will be selected from among the bidders, and a request for attendance at the bid opening will be made using the "Bid Opening Observer Request Form (Form No. 6)".
If the selected bid opening observer intends to attend by proxy, a "Proxy Form (Form No. 7)" must be submitted.
If the bid opening observer does not appear at the scheduled time, a Nara City employee unrelated to the bidding process will serve as the bid opening observer.
After the bid opening, the observer or their proxy will be asked to confirm that the bid opening was conducted fairly and properly using the "Bid Opening Confirmation Form (Form No. 8)".
10. Invalid Bids
Bids that fall under any of the following categories will be considered invalid:
* Bids by persons not eligible to participate in the bidding.
* Bids without signature or name and seal.
* Bids lacking the bid amount or the title of the work.
* Bids where necessary information cannot be confirmed due to errors or omissions in important characters.
* Bids where the amount written in the bid form has been added or corrected.
* Two or more bids for the same item submitted by the same bidder.
* Bids dated on a day other than the bid opening date.
* Bids where acts that hinder the fair execution of bidding have been recognized.
* Bids not submitted by mail, such as hand-delivery to the Personnel Division, or bids that did not arrive by the deadline.
* Other bids that violate the bidding conditions.
11. Notes on Bidding
Bids submitted by those who have made false applications regarding bidding qualifications, etc., will be invalidated, and if the successful bidder was the one who submitted the invalid bid, the award decision will be canceled.
All costs incurred by the bidder in connection with this bid shall be borne by the bidder.

Bidding Documents
Regarding the public competitive bidding for "Nara City Staff Stress Check Services (FY Reiwa 8)", in addition to the provisions of the Nara City Contract Regulations (Showa 40, Nara City Contract Regulations No. 43) and related laws and regulations, these bidding documents shall apply. Companies and organizations wishing to participate in the bidding (hereinafter referred to as "Operators") shall read the following matters carefully and participate in the bidding.
1. Project Overview
Based on the Industrial Safety and Health Act, this project aims to prevent mental health issues among employees by promoting their awareness of stress through stress level checks and by improving the work environment to create a more comfortable workplace.
2. Project Scope
(1) Project Details: As per Attached Document 1 "Nara City Staff Stress Check Services Specifications (FY Reiwa 8)"
(2) Project Implementation Period: From the date of contract conclusion to March 31, Reiwa 9
(3) Work Execution and Delivery Location: Health Management Office, 6th Floor, Central Building, Nara City Hall
3. Contract Method
(1) Contract Name: Nara City Staff Stress Check Services Contract (FY Reiwa 8)
(2) Contract Type: Service Contract (Unit Price Contract) *Please note that bidding is conducted for the total amount.
