Items for Competitive Bidding: Reiwa 8 Stress Check Services Contract (Unit Price Contract) (Electronic Bidding Applicable) 2. Bidding Method: This is an applicable case for bidding and document submission via the electronic procurement system. Those who find it difficult to use the electronic procurement system may substitute it with paper bidding by submitting a request for paper bidding. 3. Qualifications Required for Competitive Bidding Participants: (1) Must not fall under the provisions of Article 70 of the Public Accounting Act. Minors, persons under guardianship or assistance who have obtained necessary consent for contract conclusion fall under special circumstances in the same article. (2) Must not fall under the provisions of Article 71 of the Public Accounting Act. (3) Must be classified in grade C or D for "Provision of Services, etc." in the Reiwa 7/8/9 Ministry of Land, Infrastructure, Transport and Tourism Competitive Bidding Qualifications (Uniform Qualifications for All Ministries) and possess competitive bidding qualifications in the Kanto-Koshinetsu region. (4) Must not be subject to bid suspension by the Director-General of the Secretariat, Ministry of Land, Infrastructure, Transport and Tourism, from the submission deadline of the bid participation application until the bid opening. (5) If using the electronic procurement system, must have obtained electronic certification. (6) Must have received the bidding explanation document and specifications. Acquisition method is "Electronic Procurement System" or "4. Location for Contract Terms." When obtaining the bidding explanation document from the electronic procurement system, accurately enter the required information and be sure to check the box indicating "Desire to receive update notification emails when corrections or cancellations are made to downloaded cases." (7) Must have proven that the requirements specified in the specifications are met. (8) Must have pledged according to Attachment 1 "Pledge Regarding Exclusion of Yakuza." (9) Must not be within 5 years from the date of violation of the Labor Dispatch Act (excluding provisions of Chapter 3, Section 4) or any order based on these provisions (including guidance) or receiving a disposition (including guidance). (Excluding cases where corrective guidance has been received for violations of these provisions and correction has been completed by the submission of bid participation related documents.) Invalid Bids: Bids submitted by those who do not possess the required qualifications for bid participation stated in this announcement, and bids that violate the bidding conditions, will be deemed invalid. 9. Amount Stated in Bid Form: (1) For bid determination, the bid price will be the amount stated in the bid form plus 10% of that amount (if there is a fraction of less than one yen, it will be rounded down). Therefore, bidders must write the amount equivalent to 100/110 of their estimated contract value in the bid form, regardless of whether they are taxable or exempt from consumption tax and local consumption tax. (2) For unit price contracts, the total amount obtained by multiplying the unit price by the planned quantity must be stated. 10.
