1. Reiwa 8 Postal General Competitive Tender Implementation Guidelines
We will conduct a general competitive tender by mail as follows. Those who wish to participate should submit the necessary documents in accordance with these guidelines.

1. Matters for Tender
(1) Title: Kadoma City National Health Insurance Specific Health Checkup Participation Rate Improvement Countermeasure Questionnaire Consignment
(2) Performance Location: Location provided by the contractor
(3) Outline: The following services
a. Analysis of current situation and identification of issues
b. Creation of a list of target individuals for the questionnaire for those who have not undergone specific health checkups
c. Setting of questionnaire items
d. Creation and mailing of questionnaire request notices
e. Provision of incentives (rewards) to questionnaire respondents
f. Compilation, analysis, and reporting of questionnaire results
(4) Contract Period: From the contract date to March 31, Reiwa 9
(5) Other: Tenders for this consignment will be conducted with the disclosure of the estimated price.
No minimum reserve price will be set.
Estimated Price: 5,000,000 yen (excluding consumption tax and local consumption tax)
2. Qualifications Required for Tender Participants: Those who meet all of the following requirements and whose qualifications have been confirmed.
(1) Must not fall under the provisions of Article 167-4 of the Local Autonomy Act Enforcement Order.
(2) Must not have filed for commencement of rehabilitation proceedings or have had such proceedings filed against them under Article 21, Paragraph 1 or 2 of the Civil Rehabilitation Act (Heisei 11, Law No. 225).
However, those who have received a decision to commence rehabilitation proceedings under Article 33, Paragraph 1 of the same Act shall be deemed not to have filed for commencement of rehabilitation proceedings or not to have had such proceedings filed against them if a decision to approve the rehabilitation plan under Article 174, Paragraph 1 of the same Act has become final.
(3) Must not have filed for commencement of rehabilitation proceedings or have had such proceedings filed against them under Article 17, Paragraph 1 or 2 of the Corporate Rehabilitation Act (including those under Article 30, Paragraph 1 or 2 of the former Corporate Rehabilitation Act (Showa 27, Law No. 172) as amended by the same Act, which are still applicable according to the provisions of Article 2 of the Supplementary Provisions of the same Act).
However, those who have received a decision to commence rehabilitation proceedings under Article 41, Paragraph 1 of the same Act (including decisions under the former Act for old rehabilitation cases) shall be deemed not to have filed for commencement of rehabilitation proceedings or not to have had such proceedings filed against them if a decision to approve the rehabilitation plan under Article 199, Paragraph 1 of the same Act (including decisions under the former Act for old rehabilitation cases) has become final.
(4) Must not be subject to bid suspension measures under the Kadoma City Construction Work, etc. Tender Participation Suspension Guidelines (implemented December 6, Heisei 18), or not meet the measure requirements listed in the attached table of the same guidelines, regardless of registration in the city's list of eligible tenderers.
(5) Must not be subject to bid exclusion measures under the Kadoma City Public Works, etc. Anti-Organized Crime Group Measures Guidelines (implemented June 1, Heisei 24), or not meet the measure requirements listed in the attached table of the same guidelines, regardless of registration in the city's list of eligible tenderers.
(6) Must not be a person who is deemed to be significantly inappropriate to participate, similar to the requirements listed in the preceding items.
(7) Must be registered in the "7 Other Services - a. Planning/Support Services, Research, etc." category of the city's eligible tenderers list for general consignment and goods, etc., for Reiwa 8.
(8) Must have entered into and faithfully performed contracts for services related to the planning and support of national health insurance data health plans or national health insurance recipient encouragement services with the national government or other local public entities with a contract amount equal to or greater than the estimated price of this work (5,500,000 yen including tax) from April 1, Heisei 28, to the application deadline, or with this city with a contract amount equal to or greater than half of the estimated price of this work (2,750,000 yen including tax) during the same period.
3. Tender Participation Application and Tender Procedures: Those who wish to participate in this tender shall apply for participation and submit their tender by mail in accordance with the date, place, and method specified below.

b. Distribution Period and Time: From the date of notification until August 21, Reiwa 8 (Friday) (excluding Sundays, Saturdays, and national holidays as defined by the Act on National Holidays (Showa 23, Law No. 178)), from 9:00 AM to 5:30 PM (excluding 12:00 PM to 12:45 PM).
c. Distribution Location: Kadoma City Health and Welfare Center, 4th Floor, 14-1 Midosuji-cho, Kadoma City, Kadoma City Health and Welfare Department, Health Promotion Division, Adult Health Group.
(2) Inquiries Regarding Specifications: If you have any questions regarding the specifications, please send your inquiry using the Inquiry/Response Form (Form C) to the contact person listed in (a) below during the period specified in (a) below, via FAX or email. After sending by FAX or email, please contact us by phone to confirm that it has been sent.

c. Responses to Inquiries: Responses to inquiries will be posted on the city's website (https://www.city.kadoma.osaka.jp/) on August 19, Reiwa 8 (Wednesday). However, if there are no inquiries, no posting will be made.
(3) Submission Method, etc.: Those who wish to participate in the tender must mail the submission documents to the mailing address specified in (c) below within the mailing period specified in (b) below, using the method specified in (a) below.
a. Mailing Method: Must be either general registered mail or simple registered mail. Other mailing methods will not be accepted.
b. Mailing Period: From the date of notification until August 21, Reiwa 8 (Friday) (arrival deadline is the same day).
Submission documents mailed outside of this period will not be accepted for any reason.

(i) General Competitive Tender Participation Application Form (Form A)
(ii) Tender Form (Form 1)
(iii) Copy of documents confirming fulfillment of the conditions in 2(8) (e.g., contract documents).
e. Submission Method and Tender Method: The envelope containing the submission documents should generally conform to the following specifications. The envelope for the tender form (hereinafter referred to as the "inner envelope") shall be of size Nagagata 4 (90mm x 205mm), and the envelope for mailing tender participation related documents (hereinafter referred to as the "outer envelope") shall be of size Nagagata 3 (120mm x 235mm) or Kakugata 2 (240mm x 332mm).
(i) The inner envelope shall contain the tender form (Form 1), be sealed with adhesive, and the front of the envelope shall be labeled with the tender title, the bidder's trade name or name, the representative's title and name, and the warning "Strictly Unopened." It should also be marked "Tender Enclosed" in red ink and sealed with the representative's seal. Alternatively, the "Inner Envelope Label" can be created and attached instead of writing directly.
Note: Tenders where the title and sender name on the inner envelope for mailing the tender form differ from the title and sender name on the enclosed tender form will be invalid.
(ii) The outer envelope shall contain the General Competitive Tender Participation Application Form (Form A), the submission documents in 3(3)d(iii) above, and the inner envelope. It should be sealed with adhesive, the mailing address in 3(3)c should be written on it, and it should be marked "Tender Documents Enclosed" in red ink. The tender title, opening date, bidder's address, trade name or name, and representative's title and name should be written on the front and the envelope should be sealed.
However, if the necessary information is not filled in or the necessary documents are not enclosed, the participation application may not be accepted.
(iii) Mailing shall be done via the outer envelope using only general registered mail or simple registered mail.
(iv) For the contract amount, the amount obtained by adding 10% of the bid amount to the bid amount (and rounding down to the nearest yen if there are fractions of less than one yen) will be the contract amount. Therefore, tenderers must write in the tender form the amount equivalent to 100/110 of their estimated contract desired amount, regardless of whether they are a taxable business operator for consumption tax and local consumption tax.
(v) The number of tenders for this tender is one. Tenders submitted with multiple tender forms for the same item will be invalid.
(vi) Please keep the "Sender's Copy" carefully until after the tender opening.
The delivery status of the mail can be checked by calling the post office or by visiting the post office website using the acceptance number indicated on the mail receipt.
(vii) The inner envelope containing the submitted documents will be stored unopened until the tender opening time.
(viii) The cost of preparing the documents shall be borne by the tender participant.
(ix) Submitted documents mailed will not be returned.
(4) Tender Participation Qualification Confirmation Results: Tender participation qualification will be reviewed based on the submitted application documents. The results of the tender participation qualification will be published on the city's website (https://www.city.kadoma.osaka.jp/) only if the number of applicants recognized as qualified reaches one. Applicants who are not recognized as qualified will be individually notified by FAX or email with reasons. This public announcement and notification will serve as the qualification confirmation result notification for those recognized as qualified. The number of qualified applicants and unqualified applicants, as well as their trade names or names, will not be published.
a. Publication Date: August 26, Reiwa 8 (Wednesday).
b. If a tenderer loses their qualification to participate by the tender opening date, their tender will not be opened.
c. If the number of qualified applicants is less than one after the qualification confirmation, the tender participation qualification results will not be published.
4. Bid Bond: Exempted in accordance with Article 7, Item 3 of the Kadoma City Contract Regulations (Showa 39, Regulation No. 7).
5. Withdrawal of Tender Participation Application: If you wish to withdraw your tender participation application after mailing the tender documents, please submit the Tender Participation Application Withdrawal Form (Form E) in person or by mail to the tender document mailing address by the tender opening time. If mailing by the tender opening time is not possible, please send it by FAX or email by the tender opening time and submit the original later. Please note that you cannot change or withdraw only the mailed tender form without withdrawing the tender participation application.
6. Tender Opening:
(1) The tender opening for this tender will be conducted at the date and time and location specified below, with at least one witness or a city employee not involved in the tender procedures present.
a. Date and Time: August 31, Reiwa 8 (Monday) at 2:00 PM.
b. Location: Tender Room, 3rd Floor, Main Building, Kadoma City Hall, 1-1 Nakamachi, Kadoma City.
(2) Selection of Witnesses: Witnesses will be selected from among tender participants who have sent the Postal Tender Opening Witness Application Form (Form G) by email or FAX within the period specified in (a) below, up to two individuals in order of receipt.
a. Witness Application Period: From the public announcement of tender qualification confirmation until August 27, Reiwa 8 (Thursday). Phone contact after sending: From 9:00 AM to 5:30 PM (excluding 12:00 PM to 12:45 PM) on days other than Sundays, Saturdays, and national holidays.
b. Witness Application Submission Address: Same as 15.
c. If a person other than the tenderer wishes to be a witness, they must bring a Witness Power of Attorney (Form H) to the tender opening meeting and submit it.
d. Selected witnesses will generally be contacted by phone.
(3) Determination of Successful Bidder:
a. In this tender, the successful bidder will be the one who submits the lowest bid within the range of the estimated price, or, if a minimum reserve price has been set in advance in accordance with Article 167-10, Paragraph 2 of the Local Autonomy Act Enforcement Order, the one who submits the lowest bid at or above the minimum reserve price.
b. If there are two or more bidders with the same lowest bid, the successful bidder will be determined by lottery.
7. Publication of Tender Results, etc.:
(1) The results of the successful bid will be notified only to the successful bidder. For other tender participants, the results will be published as follows:
a. Publication Location: Kadoma City Hall Annex (Kadoma Nakamachi Building), Kadoma City Information Corner, 1-19 Nakamachi, Kadoma City.
b. Other: The results will also be promptly posted on the city's website (https://www.city.kadoma.osaka.jp/).
8. Invalid Tenders: Tenders falling under any of the following items will be invalid.
