TENDER ANNOUNCEMENT
We hereby announce a general competitive tender for the General Periodic Health Examination Services Contract (Unit Price Contract) as follows.

Matters for Competitive Tender: Subject: General Periodic Health Examination Services Contract (Unit Price Contract). Details: As per specifications. 2. Service Period: As per specifications. 3. Delivery Location: Location designated by the Authorized Officer for Expenditure (as per specifications). 4. Tender Method: The bid amount shall be for the total price. When determining the successful bidder, the successful bid price will be the amount obtained by adding 10% of the amount written on the bid form to that amount (if there is a fraction of less than 1 yen, that fraction shall be rounded down). Therefore, bidders, regardless of whether they are taxable or non-taxable for consumption tax and local consumption tax, must write the amount equivalent to 100/110 of their estimated contract amount on the bid form. 5. Qualifications for Participation in Competition, etc.: (1) Must not fall under the provisions of Article 70 of the Public Accounting Act. Minors, persons under guardianship or assistance who have obtained the necessary consent for contract conclusion fall under the exception for special reasons. (2) Must not fall under the provisions of Article 71 of the Public Accounting Act. (3) Must be graded A, B, or C in "Services, etc." in the Reiwa 07/08/09 Ministry of Health, Labour and Welfare Competitive Participation Qualification (All-Ministry Unified Qualification) and have qualification to participate in competition in the Tohoku region. (4) For those to whom systems such as social insurance (Employees' Pension Insurance, Health Insurance (managed by the Japan Health Insurance Association), Seamen's Insurance, National Pension, Workers' Accident Compensation Insurance, and Employment Insurance) apply, they must be enrolled in these systems and have no arrears in premiums for the most recent 2 years (for Workers' Accident Compensation Insurance and Employment Insurance, 2 insurance years). (5) Must not be subject to designation suspension measures by the Ministry of Health, Labour and Welfare. (6) Must be recognized as not having provided false information in the qualification examination application or attached documents. (7) Must be recognized as not having a critically deteriorated financial condition or creditworthiness. (8) Must possess other qualifications determined by the Authorized Officer for Expenditure based on Article 73 of the Public Accounting Act. 6. Use of Electronic Procurement System: This project will be conducted using the electronic procurement system. However, those who have difficulty using the electronic procurement system may switch to the paper-based tender method upon request to the contracting authority. 7. Invalid Bids: Bids made by persons who do not possess the qualifications required for participation in this tender, bids by those who have failed to fulfill the obligations required of bidders, and other bids that violate the tender conditions are invalid. 10. Other: (1) All expenses (delivery, etc.) shall be borne by the supplier. (2) The successful bidder shall enter into a contract with the government. Contract exchange shall be conducted primarily through the electronic procurement system. However, if it is difficult to use the electronic procurement system, paper-based contracts may be used with the approval of the Authorized Officer for Expenditure.
