Description

Services may include but are not limited to the following: • Goal setting and action planning; • Development of daily living and life management skills; • Housing stability support; • Assistance navigating government systems and community resources; • Crisis intervention and safety planning; • Emotional support; • Providing transportation, accompaniment, and support to help young adults access appointments, services, resources, programs, and other activities identified through service planning; and • Support accessing education, employment, cultural connections, and other community- based opportunities.
