Part 1 – Expansion of HIS and PACS

1. The order is being implemented as part of investment D1.1.2 “Accelerating digital transformation processes in healthcare through further development of digital services in healthcare”, Component D “Efficiency, accessibility and quality of the healthcare system” financed by the Ministry of Health from the National Reconstruction Plan (KPO). 2. The subject of the order is the expansion of IT systems of the Independent Public Healthcare Unit in Mława. 3. A detailed description of the subject of the order can be found in annexes no. 2.1, 2.2, 2.3, 2.4 to the SWZ. 4. The contractor may submit an offer for one or more parts of the order. The Ordering Party does not limit the number of order parts that can be awarded to a single contractor. The Ordering Party reserves the necessity for the Contractor to submit offers for the entire assortment in a given part/package. 5. The Ordering Party plans to use the so-called reverse procedure, referred to in art. 139 sec. 1 of the Pzp, i.e. the Ordering Party will first examine and evaluate the offers, and then will qualify the subject of the Contractor whose offer has been rated highest, in terms of the absence of grounds for exclusion and meeting the conditions for participation in the proceedings. Pursuant to art. 139 sec. 2 of the Pzp, the Ordering Party will require the submission of a single European procurement document (JEDZ) only from the contractor whose offer has been rated highest. 6. The Ordering Party may annul the public procurement procedure before the deadline for submitting offers if circumstances arise that make further proceedings unjustified. 7. Deadline for completion of the subject of the order: by 31.05.2026. The deadline is dictated by the need to settle the targeted subsidy. In case of failure to meet the subsidy settlement deadline, the Ordering Party is obliged to return the entire targeted subsidy. 8. Payment deadline: 60 days from the date of delivery of a correctly issued VAT invoice. 9. Upon the Ordering Party's request, the Contractor whose offer has been rated highest shall submit, within the specified period, not shorter than 10 days, subject evidence valid on the day of submission, to confirm the absence of grounds for exclusion: 10. Upon the Ordering Party's request, the Contractor whose offer has been rated highest shall submit, within the specified period, not shorter than 10 days, subject evidence valid on the day of submission: 1) to confirm the absence of grounds for exclusion: a) Contractor's statements, within the scope of art. 108 sec. 1 item 5 PZP, regarding the absence of belonging to the same capital group within the meaning of the Act of February 16, 2007 on competition and consumer protection with another contractor who has submitted a separate offer, a partial offer, or statements of belonging to the same capital group along with documents or information confirming the preparation of the offer, a partial offer independently of another contractor belonging to the same capital group – the template is Annex No. 4 to the SWZ; b) information from the National Court Register regarding: art. 108 sec. 1 items 1 and 2 PZP; art. 108 sec. 1 item 4 PZP, concerning the imposition of a ban on applying for public procurement as a precautionary measure – prepared no earlier than 6 months before its submission; c) Contractor's statement on the currency of the information contained in the statement referred to in art. 125 sec. 1 PZP, regarding the grounds for exclusion from the proceedings indicated by the Ordering Party, referred to in: art. 108 sec. 1 item 3 PZP, art. 108 sec. 1 item 4 PZP, concerning the imposition of a ban on applying for public procurement as a precautionary measure, art. 108 sec. 1 item 5 PZP, concerning the conclusion of an agreement with other contractors aimed at disrupting competition, art. 108 sec. 1 item 6 PZP, 2) to confirm the fulfillment of the conditions for participation in the proceedings a) List of supplies – the template is Annex No. 8 to the SWZ.
