1. The subject of the public procurement procedure is: Collection and management of waste from all USK No. 2 facilities – 2 parts (packages), in accordance with the assortment and quantities specified in the Price Form constituting Annex No. 2 to the SWZ: 1.1 Part 1: Services for the collection and disposal of medical waste and medicines from the USK No. 2 hospital complexes in Łódź at ul. Żeromskiego 113, Plac Hallera 1, Pieniny 30, – in accordance with the Act of December 14, 2012 on waste (Journal of Laws of 2023, item 1587 - consolidated text, as amended) with codes 18 01 01, 18 01 02*, 18 01 03*, 18 01 06*, 18 01 08*, 18 01 09, 18 01 10* according to the Regulation of the Minister of Climate of January 2, 2020 on the waste catalog (Journal of Laws of 2020, item 10) – in accordance with the requirements specified in Annex No. 1a – Description of the subject of the order; The Contracting Authority requires the content of the offer to comply with the proximity principle expressed in Article 20(3), 5-6 of the Waste Act, i.e., it requires that the disposal of infectious medical waste from part 1 takes place in the same voivodeship in which the waste was generated, or outside the voivodeship, but closest to the place of its generation. In the absence of a facility for the disposal of such waste in a given voivodeship or if existing facilities do not have spare capacity, the Contracting Authority permits the disposal of infectious medical waste in a voivodeship other than the one in which it was generated, in the closest located facility. The Contracting Authority will verify compliance with the proximity principle at the offer evaluation stage. 2. The Contracting Authority plans to award the contract referred to in Art. 214 sec. 1 item 7 of the Act. 3. The Contracting Authority requires the Contractor or Subcontractor to employ persons performing the following activities in the scope of order execution on the basis of an employment contract: collection and transport of waste
