Leki różne: Enoxaparinum Natricum r-r do wstrz. 20 mg / 0,2 ml amp.strzyk. 0,2 ml; 40 mg /0,4 ml amp.strzyk. 0,4 ml; 60 mg / 0,6 ml amp.strzyk. 0,6 ml; 80 mg / 0,8 ml amp.strzyk. 0,8 ml; 100 mg / 1 ml amp.strzyk. 1 ml; 120 mg / 0,8 ml amp.strzyk. 0,8 ml; 150 mg / 1 ml amp.strzyk. 1 ml 2. Szczegółowy opis przedmiotu zamówienia, wymagania Zamawiającego określono w Formularzu cenowo – ofertowym – Załączniku nr 1 do SWZ. 3. Zakres zamówienia obejmuje dostawę przedmiotów zamówienia w ilościach wyszczególnionych w Formularzu cenowo – ofertowym na okres: - Część nr 1 – 12 miesięcy. 4. Zamawiający nie dopuszcza składania ofert równoważnych. 5. TERMIN WYKONANIA ZAMÓWIENIA: 5.1 Zakup i sukcesywna dostawa przedmiotów zamówienia: - Część nr 1 - przez 12 miesięcy, licząc od dnia zawarcia umowy. 5.2 Zamawiający w Części nr 1 dopuszcza możliwość zmiany terminu obowiązywania umowy zawartej z Wykonawcą w przypadku nie zrealizowania w okresie jej trwania całości przedmiotu umowy. W takim przypadku zmiana umowy wymaga podpisania aneksu, z tym że Zamawiający jednostronnie zawiadamia Wykonawcę o proponowanym przedłużeniu umowy przed upływem terminu na jaki została ona zawarta.
