1. Leki różne – 211 pozycji w części 2. Szczegółowy opis przedmiotu zamówienia, wymagania Zamawiającego określono w Formularzu cenowo – ofertowym – Załączniku nr 1 do SWZ. 3. Zakres zamówienia obejmuje dostawę przedmiotów zamówienia w ilościach wyszczególnionych w Formularzach cenowo – ofertowych na okres: - dla Części nr 1 – 24 miesięcy od dnia zawarcia umowy. 4. Zamawiający nie dopuszcza składania ofert równoważnych. 5. TERMIN WYKONANIA ZAMÓWIENIA: 5.1 Przedmiot zamówienia w zakresie Części nr 1 będzie realizowany w formie sukcesywnych dostaw przez okres 24 miesięcy od dnia zawarcia umowy. 5.2 W przypadku, gdy w okresie obowiązywania umowy nie zostanie zrealizowany w całości przedmiot zamówienia w odniesieniu do Części nr 1, Zamawiający zastrzega sobie możliwość przedłużenia okresu obowiązywania umowy.
