The detailed description of the subject of the order and the Contracting Authority's requirements are specified in the price-offer forms – Annexes No. 1A and 1B, which are an integral part of the SWZ. 3. The Contracting Authority requires the offered subject of the order to: 3.1. meet the requirements described in the price-offer forms, 3.2. meet the requirements described in the SWZ, 3.3. be covered by the Act of May 12, 2011, on the reimbursement of medicines, specialized nutritional foods, and medical devices – this applies to the subject of the order covered by reimbursement. 4. The scope of the order includes the delivery of the subject of the order in the quantities specified in the price-offer forms for a period of: – 24 months. 5. ORDER COMPLETION DEADLINE: 5.1 Purchase and successive delivery of the subject of the order: for 24 months, starting from the date of contract conclusion. 5.2 The Contracting Authority allows for the possibility of changing the validity period of the contract concluded with the Contractor if the entire subject of the contract is not completed within its duration. In such a case, the contract amendment requires signing an annex, with the Contracting Authority unilaterally notifying the Contractor of the proposed contract extension before the expiry of the period for which it was concluded. 6. The Contracting Authority does not allow the submission of equivalent offers.
