Supply of X-ray apparatus as part of the investment task "Investment Purchases for the Municipal Care Center"

1. The subject of the order is the supply, installation and commissioning of X-ray apparatus, along with the prior reconstruction of selected rooms. 2. The subject of the order includes: 1.1. Supply, installation, connection, commissioning, qualification of X-ray apparatus, quality control equipment, computer equipment 1.2. Supply of disinfection devices and supply of PACS/RIS software, implementation of PACS/RIS software and its full integration with the Contracting Authority's HIS system (AMMS by Asseco). 1.3. Reconstruction (adaptation) of rooms for a specific model of apparatus supplied by the Contractor (including in particular): • development of design documentation for the reconstruction of the existing X-ray room, preparation of the design for fixed shielding, • modernization of fixed shielding. 1.4. On behalf of the Contracting Authority, registration of the X-ray apparatus and obtaining permission for its use (and the use of the adapted rooms) from the relevant authorities, institutions and services by administrative decision. 1.5. After-sales service of X-ray apparatus, quality control equipment, computer equipment, disinfection devices; including training, warranty service, etc. 3. A detailed description of the subject of the order is included in Annex No. 2 to the SWZ together with Annex A, and should be read together with any changes to the content of the SWZ, resulting for example from the Contracting Authority's answers to the Contractors' questions. 4. The Contractor must offer the subject of the order in compliance with the Contracting Authority's requirements specified in the SWZ, and is obliged to indicate in Annex No. 1 the type/model, name (company) of the manufacturer of the offered X-ray apparatus, quality control equipment, disinfection devices, PACS system and to attach to the offer the subject evidence, specified in Chapter V of the SWZ, serving to confirm the compliance of the offered supplies with the requirements specified in the description of the subject of the order. Verification of the compliance of the offered subject of the order will be carried out based on the information contained in Annex No. 1 to the SWZ, including in particular based on the type/model, name (company) of the manufacturer of the offered subjects of the order and on the subject evidence referred to in Chapter V of the SWZ. 5. Definition of the subject of the order according to the Common Procurement Vocabulary (CPV): • 33111000-1 – X-ray apparatus, • 33111800-9 – Diagnostic X-ray system, • 45453000-7 – Renovation and repair work, • 45450000-6 – Finishing construction works, other 6. The Contracting Authority does not allow for the submission of partial offers; 6.1. Before commencing the proceedings, the Contracting Authority conducted preliminary market consultations regarding the purchase of X-ray apparatus and adaptation of rooms, which indicate that Contractors have the free possibility to carry out adaptation works of the rooms where the apparatus is to be installed. 6.2. The lack of division into parts will not lead to logistical difficulties, and thus will not lead to a restriction of competition, especially for SMEs. In reality, it is the division into parts that would lead to logistical difficulties for the Contracting Authority and potential Contractors, where the work of the adaptation contractor and the X-ray apparatus supply contractor would need to be coordinated at the installation site. 6.3. Due to the specifics of installing X-ray apparatus, the adaptation works related to it for full compatibility of supply and construction works in terms of purpose and result must be carried out by one Contractor. 7. Before submitting an offer, the Contracting Authority requires Contractors to conduct a site visit to determine the optimal conditions for carrying out adaptation works, installation and assembly works, including any construction works. 7.1. A site visit can be made daily between 8:00 AM and 1:00 PM on working days (excluding Saturdays and Sundays/holidays) at the Municipal Care Center until the offer submission deadline. 7.2.
