Estimated quantities of products and their description, along with necessary requirements, are included in the offer form (Annex 1 to SWZ, hereinafter also referred to as FO), unit price form (Annex 2 to SWZ, hereinafter also referred to as FCJ), and required parameters form (Annex 2A to SWZ, hereinafter also referred to as FPW). 3. The Ordering Party requires the offered products to be medical devices - within the meaning of the Act of April 7, 2022, on Medical Devices (Journal of Laws 2024.1620 as amended), hereinafter "UoWM"), introduced to the market and used in the territory of the Republic of Poland in accordance with applicable regulations.
