1. Przedmiotem zamówienia jest wykonanie usługi polegającej na dowozie dzieci niepełnosprawnych z gminy Goleniów do Ośrodka Rehabilitacyjno-Edukacyjno-Wychowawczego Profi-Med Sp. z o. o. przy ul. Marii Konopnickiej 10A, zwanego dalej „OREW w Goleniowie. 2. Świadczona usługa polegać będzie na przewiezieniu dzieci z ich miejsc zamieszkania do OREW w Goleniowie oraz na odwiezieniu ich po zajęciach z OREW w Goleniowie do ich miejsc zamieszkania, łącznie z zapewnieniem opieki nad tymi dziećmi podczas transportu oraz zgodnie z załącznikiem nr 1.1. do SWZ - Dzienną kalkulacją transportu, a także zgodnie z załącznikiem nr 9 do SWZ - Harmonogramem transportu. Załącznik nr 1.1. do SWZ i załącznik nr 9 do SWZ zawierają optymalną kolejność przewozu, dostosowaną do rozkładu zajęć w OREW w Goleniowie. Transport odbywać się będzie w podziale na co najmniej dwie trasy realizowane jednocześnie. 3. Maksymalna dzienna praca przewozowa wynosi 141 km, lecz trasa przejazdu zależy od obecności dzieci w danym dniu. 4. dalszy szczegółowy opis przedmiotu zamówienia określono w dokumentach zamówienia.
