Description

West Dunbartonshire Health and Social Care Partnership (WDHSCP) is seeking to establish a Flexible Framework for the provision of additional support services for adults aged 18–65. Services will support individuals with a range of needs including learning disabilities, autism, complex needs, physical disabilities, cognitive impairments, sensory impairments and mental health conditions. The Framework will operate on a flexible basis, allowing new providers to apply for admission throughout its term. Provision of additional support services for adults aged 18–65 with learning disabilities, including those with complex needs and behaviours that challenge. Services will be person centred, outcomes focused and delivered in line with Self Directed Support Option 2 and Option 3. Providers will support individuals to live independently within their communities and work in partnership with the HSCP and other agencies. Services must be capable of delivery on a 24 hours a day, 7 days a week, 365 days a year basis where required. The duration of the framework exceeds four years due to the complex and long term nature of the services being delivered. Continuity of care for vulnerable individuals, workforce stability and the need for providers to invest in sustainable service delivery models require a longer term arrangement. The flexible structure of the framework ensures ongoing competition and responsiveness to changing needs. Additional information: This lot forms part of a flexible framework which will remain open to new entrants throughout its duration. Provision of additional support services for adults aged 18–65 with physical disabilities. Services will support individuals to maximise independence and achieve personal outcomes, including delivery of personal care, mobility support and community engagement. Services will be delivered in line with Self Directed Support Option 2 and Option 3 and must reflect a person centred, outcomes based approach. Services must be capable of delivery on a 24 hours a day, 7 days a week, 365 days a year basis where required. The duration of the framework exceeds four years due to the complex and long term nature of the services being delivered. Continuity of care for vulnerable individuals, workforce stability and the need for providers to invest in sustainable service delivery models require a longer term arrangement. The flexible structure of the framework ensures ongoing competition and responsiveness to changing needs. The flexible structure of the framework ensures ongoing competition and responsiveness to changing needs. The duration of the framework exceeds four years due to the complex and long term nature of the services being delivered. Continuity of care for vulnerable individuals, workforce stability and the need for providers to invest in sustainable service delivery models require a longer term arrangement. The flexible structure of the framework ensures ongoing competition and responsiveness to changing needs.
