Description

*FUNDING ACTIVITY*

The objective of assertive outreach is to seek out people sleeping rough wherever they may be living and support a transition to appropriate housing and/or support as quickly as possible. A core element of assertive outreach is to use persistent and purposeful engagement rather than waiting for people sleeping rough to request a service or for a referral, assertive outreach workers proactively engage people, building trust and relationships over time to connect them with services they need.

Assertive outreach promotes and delivers service assistance to people experiencing entrenched forms of homelessness and who are disengaged with the service system because of repeated system failures compounded by histories of trauma. Assertive outreach is a support-only program which also includes flexible brokerage.
