Dit is de functieVanuit jouw expertise op het terrein van jeugdhulp help je bij het tot stand komen van het gezinsplan en zet je zo nodig professionele ondersteuning in. Samen met de inwoner en zijn omgeving werk je aan een betere toekomst. Je hebt oog voor opvoedingsproblematiek, psychiatrie en andere problematieken. Je kijkt systemisch, je hebt lef en denkt verder dan de gebaande paden. Dit ga je doen? Je gaat bij inwoners thuis op bezoek om een ?keukentafelgesprek? te voeren. Hierbij is het van groot belang dat het hele systeem wordt betrokken in de probleemanalyse. Je onderzoekt op welke gebieden de inwoner hulp nodig heeft en hebt een brede kijk op het gezinssysteem en de verschillende levensgebieden.? Je onderzoekt met je specialistische kennis vervolgens de jeugdhulpplicht die de gemeente per situatie heeft volgens de Jeugdwet. ? Je ondersteunt een gezin/familie om een ondersteunings-/familiegroepsplan op te stellen. ? Je werkt integraal, zoekt de verbinding op en stemt af met andere professionals in de informele en formele zorg. ? Je rapporteert en indiceert de inzet aan professionele zorg.? Je sluit aan bij planbijeenkomsten om actief mee te denken vanuit jouw expertise.? Je bent indien nodig ook perspectiefregisseur. Dat betekent dat je het hulpaanbod co?rdineert, plant, doelen monitort en evalueert. ? Je voert onderzoek uit naar de doel- en rechtmatigheid van maatwerkvoorzieningen.? Je geeft vanuit de praktijk input aan de beleidsadviseurs over kwalitatief goede en effici?nte voorzieningen jeugdhulp en ziet verbetermogelijkheden.? Je voert regie bij onveilige situaties of in een crisissituatie en schaalt op of grijpt in wanneer dit nodig is. Je bent in staat om de veiligheid van de jeugdige voorop te stellen. Je kunt de inschatting maken of er sprake is van ontwikkelingsbedreiging van de jeugdige en stelt veiligheidsplannen op om deze te waarborgen.
