Naprawa cysto-ureterofiberoskopów oraz ureterenoskopu 1. Przedmiotem zamówienia jest usługa polegająca na naprawie 8 szt. cysto-ureterofiberoskopów oraz 1 szt. ureterenoskopu stanowiących wyposażenie Zamawiającego, wraz z wykonaniem wszystkich czynności niezbędnych do przywrócenia sprawności technicznej i użytkowej sprzętu, w tym w szczególności: diagnostyki uszkodzeń, robocizny, wymiany niezbędnych części i elementów, wykonania testów sprawności oraz sporządzenia dokumentacji serwisowej/protokołu naprawy. 2. Wykaz cysto-ureterofiberoskopów oraz ureterenoskopu objętych naprawą został zamieszczony w Załączniku nr 2 do SWZ. 3. Zamawiający przewiduje prawo opcji. Prawo opcji polega na możliwości zlecenia Wykonawcy, w okresie obowiązywania umowy, naprawy maksymalnie kolejnych 8 szt. cysto-ureterofiberoskopów oraz 4 szt. ureterenoskopów, na warunkach określonych w SWZ, Formularzu ofertowo-cenowym oraz projektowanych postanowieniach umowy. 4. Zakres podstawowy zamówienia obejmuje naprawę 8 szt. cysto-ureterofiberoskopów oraz 1 szt. ureterenoskopu. Zakres opcjonalny obejmuje naprawę od 1 do maksymalnie 8 dodatkowych szt. cysto-ureterofiberoskopów oraz 4 szt. ureterenoskopów. Zamawiający może skorzystać z prawa opcji w całości, w części albo nie skorzystać z niego w ogóle. 5. Skorzystanie z prawa opcji nastąpi poprzez złożenie Wykonawcy jednostronnego oświadczenia/zlecenia przez Zamawiającego, w formie pisemnej lub elektronicznej, określającego liczbę cysto-ureterofiberoskopów oraz ureterenoskopów objętych opcją oraz termin i sposób ich przekazania do naprawy.
