Description

The High Intensity Service provide a non-medicalised approach and support for those people who attend A&E a minimum of 5 times per year, who are over the age of 18 and reside in the area covered by Stockport. Working with the Emergency Department at Stepping Hill Hospital the service will identify those at greatest risk of 999 calls, A&E attendance, and non-elective admissions. The HIU Service in Stockport is focused and targeted at people who present with mental health needs and conditions. With a trained and experienced workforce, the HIU service deliver a humane and holistic 1:1 casework approach to identify triggers, gaps in support and unmet social needs. Coach and activate using a strengths-based approach.

This contract will be for 3-years from 1st April 2025 - 31st March 2028.
The total contract value will be £370,543.

The awarding of this contract is subject to the Health Care Services (Provider Selection Regime) Regulations 2023.
