1.1. Przedmiotem zamówienia jest Odbiór, transport i utylizacja odpadów medycznych. Odbiór i unieszkodliwianie odpadów medycznych o kodach 180101, 180102,180103, 180104, 180106, 180107, 180109, 180182 odbywać się będzie wg opisu przedstawionego formularzu asortymentowo-cenowym i na warunkach określonych w projekcie umowy będącej załącznikiem do niniejszego opisu. 1.2. Miejscem odbioru odpadów przez Wykonawcę od Zamawiającego jest Powiatowe Centrum Zdrowia S.A. w Kluczborku ul. M. Skłodowskiej - Curie 23, oraz Zakład Opiekuńczo - Leczniczy w Wołczynie ul. Kościuszki 1. 1.3. .Zbiórkę odpadów z oddziałów i innych komórek organizacyjnych Szpitala dokonuje Zamawiający i dostarcza odpady do magazynu odpadów medycznych. Załadunek odpadów z magazynu odpadów medycznych na środek transportu Wykonawcy dokonuje/ą pracownik/pracownicy Wykonawcy. Odbiór odpadów odbywać się będzie z pomieszczenia wyznaczonego do czasowego magazynowania odpadów z częstotliwością co najmniej trzy razy w tygodniu - poniedziałek, środa, piątek i w razie potrzeby na zgłoszenie telefoniczne Zamawiającego, w godz. 8.00 -14.00. Zamawiający zastrzega możliwość zmian terminów odbiorów odpadów w zależności od bieżących potrzeb. 1.4. Zamawiający zobowiązany jest do każdorazowego ważenia i ewidencjonowania odpadów. 1.5. Wagę przekazywanego odpadu Zamawiający każdorazowo wpisuje na worek z odpadem medycznym oraz do zeszytu na podstawie którego zliczana jest masa odpadów. Zamawiający na tej podstawie wypisuje " Kartę przekazania odpadu w systemie BDO dla każdego rodzaju wytworzonego odpadu. 1.6. Zamawiający posiada wagę elektroniczną, która znajduję się w magazynie odpadów medycznych.
