The subject of the order is the development of complete multi-branch design and cost documentation, including obtaining a building permit and providing author's supervision for the project "Construction of a didactic and scientific building of Lublin University of Technology" named IT TOWER with a single-level underground parking lot at ul. Nadbystrzycka in Lublin. 2. The description of the Contracting Authority's needs and the characteristics of the ordered service, which is the subject of the order, includes: Description of the Subject of the Order/Description of the Contracting Authority's Needs (hereinafter OPZ), which is Annex No. 1 to the OPiW together with its annexes. List of subject-matter evidence to be submitted by the Contractor along with the Application to confirm the Contractor's compliance with the participation conditions in the proceedings. 1. In order to confirm the Contractor's compliance with the participation conditions regarding technical or professional capacity, the Contracting Authority requires the following subject-matter evidence: 1) a list of services (in electronic form), performed, and in the case of recurring or continuous services, also being performed, within the last 6 years, and if the period of business activity is shorter - within that period, including their value, subject, dates of performance and entities for whom the services were performed or are being performed, and attaching evidence specifying whether these services were performed or are being performed properly, where the evidence referred to are references or other documents prepared by the entity for whom the services were performed, and in the case of recurring or continuous services being performed, and if for a justified reason of an objective nature the contractor is unable to obtain these documents - a contractor's statement; in the case of recurring or continuous services still being performed, references or other documents confirming their proper performance should be issued within the last 3 months - the template for the list of services is Form No. 3 to the description of needs and requirements. 2) a list of persons (in electronic form) - delegated by the Contractor to perform the public contract, in particular responsible for providing services, quality control or construction supervision, along with information on their professional qualifications, entitlements, experience and education necessary to perform the public contract, as well as the scope of activities performed by them and information on the basis for having these persons at their disposal - the template for the list of persons is Form No. 4 to the description of needs and requirements. 3) a paid OC insurance policy or other document confirming that the contractor is insured against civil liability in connection with the business activity related to the subject of the order for the sum insured specified by the Contracting Authority. 2. When assessing technical or professional capacity, the Contracting Authority may, at any stage of the proceedings, consider that the Contractor does not have the required capacity if the Contractor's conflicting interests, in particular the involvement of the Contractor's technical or professional resources in other business ventures of the Contractor, may negatively affect the performance of the order. STATEMENTS AND DOCUMENTS REQUIRED TO BE SUBMITTED WITH THE APPLICATION FOR ADMISSION TO PARTICIPATE IN THE PROCEEDINGS/PRELIMINARY OFFER 1. The application form for admission to participate in the proceedings is Annex No. 3 to the OPiW. The Contractor shall submit with the Application: 1) a statement on the European Single Procurement Document (ESPD) form and Form No. 1 and Form No. 2 (if applicable), 2) a power of attorney or other documents confirming the authority to represent the Contractor in these proceedings or to sign the contract (if not evident from registration documents) 3) in the case of contractors jointly applying for the contract, a power of attorney or other document to represent the contractors jointly applying for the contract.
