Lot 1

1. The subject of the order is the provision of comprehensive laundry and disinfection services, as well as collection and delivery of hospital linen from pediatric and adult wards (mattresses, pillows, duvets, blankets, mops, and work clothes of the staff, owned by the Ordering Party) in the quantity and assortment presented in Annex No. 1 to the SWZ - "Assortment and Price List". 2. The detailed description of the subject of the order is contained in: Annex No. 1a - Description of the subject of the order OPZ 3. Detailed conditions and rules regarding the execution of the subject of the order are regulated in the Draft Agreement - Annex No. 4 to the SWZ "Contractual Conditions". 4. The Ordering Party requests the Contractor to indicate in the offer the parts of the order that it intends to entrust to Subcontractors and to provide the names of the Subcontractors – in accordance with art. 462 sec. 2 of the PZP Act, if they are already known. 5. If the change or resignation of a Subcontractor concerns an entity whose resources the Contractor relied on, in accordance with art. 118 sec. 1 of the PZP Act, to demonstrate compliance with the conditions for participation in the proceedings, the Contractor is obliged to demonstrate to the Ordering Party that the proposed alternative Subcontractor or the Contractor itself meets them to no lesser extent than the Subcontractor whose resources the Contractor relied on during the public procurement procedure. Art. 122 of the PZP Act applies accordingly.
