The Ordering Party indicates that, in accordance with the program standards, the number of places in an assisted living facility cannot exceed 7, and rooms in the assisted living facility should be single rooms, however, this order concerns only the caregiver service in the premises indicated by the Ordering Party. 5. The service will be provided in particular for people requiring support in daily functioning, including people with disabilities. The service is not institutional care and cannot be provided in a way that replaces the residents' ability to make decisions independently, perform daily activities, and use the resources of the local community, subject to the necessary support resulting from their individual needs. 8. The type and scope of support provided in the assisted living facility should be adapted to the individual needs of the residents, taking into account the agreements made in the documents concerning the resident's stay and support, in particular in the contract, support program, or individual self-reliance program, if such a document has been developed. 9. Within the scope of the order, the Contractor provides services in premises provided by the Ordering Party. The Contractor is not responsible for providing the premises, but is obliged to immediately inform the Ordering Party about circumstances concerning the use of the apartment, resident safety, technical condition of equipment, accessibility of the premises, and other events that may affect the quality or continuity of support. 10. The scope of the service includes, in particular: • coordination of administrative activities related to the direct functioning and management of the assisted living facility, including supervision of the proper use of the apartment by its residents; • ongoing monitoring of the social functioning and health status of the residents and responding in case of deterioration of health, cessation of medication, or occurrence of other circumstances requiring intervention; • cooperation with residents and supporting them in solving problems, in particular providing assistance and indicating ways to handle current daily, personal, and official matters; • participation in the organization and coordination of services provided in the assisted living facility and cooperation with staff, specialists, institutions, and entities involved in the implementation of support; • supporting residents in the implementation of individual self-reliance programs, support programs, or other documents defining the scope of support, as well as conducting ongoing assessment of the residents' situation determining the scope and type of support provided; • cooperation with residents' families, if justified by the resident's situation, in accordance with the scope of support and permissible under data protection regulations and confidentiality rules; • assistance in resolving conflicts between residents, including mediation and preventive actions to de-escalate tensions; • conducting activities aimed at maintaining and developing residents' independent functioning skills; • supporting residents' activities in the apartment, including learning, maintaining, or counteracting regression of the highest possible level of skills in self-care, life independence, maintaining social contacts, and fulfilling social roles; • organizing or supporting independence training, adequate to the residents' needs, in particular training in practical skills, hygiene, apartment management, financial management, meal preparation, spending free time, skills in...
