Original language: DutchHet betreft het leveren van: Chronische zuurstoftherapie aan nieuwe patiënten binnen de zuurstofconventie die, op voorschrift van de behandelende arts, bij de patiënten thuis geleverd wordt. Met name: -Langdurige zuurstoftherapie thuis met een vaste zuurstofconcentrator -Langdurige zuurstoftherapie thuis met een vaste zuurstofconcentrator en compressor om kleine draagbare flessen te vullen -Langdurige zuurstoftherapie thuis met een vaste zuurstofconcentrator, aangevuld met een draagbare zuurstofconcentrator -Langdurige zuurstoftherapie thuis met vloeibare zuurstof -Langdurige zuurstoftherapie thuis met uitsluitend een draagbare zuurstofconcentrator Bestaande patiënten van niet-gegunde leveranciers kunnen worden overgezet, tenzij de huidige leveranciers bereid zijn om op basis van onderling afgesproken voorwaarden de samenwerking voort te zetten. Deze opdracht omvat het leveren van toestel/ medische zuurstof en toebehoren; alsook het onderhoud en opvolgen/begeleiden van de patiënt volgens de richtlijnen van de conventie, indien van toepassing. Alle toebehoren, controle, onderhoud, opleiding patiënt/personeel zitten in de totaalkost vervat. Het voorzien van een medische zuurstoftank/zuurstofconcentratoren in het ziekenhuis is mogelijk indien hier nood aan is. De modaliteiten en prijs worden meegedeeld en maken deel uit van de beoordeling. De afname voor dit perceel zal voor UZ Leuven, AZ Diest en HH Leuven gebeuren bij de leveranciers die als eerste drie in de ranking staan . 50% van het volume wordt afgenomen van de inschrijver met die de hoogste totale score haalt op de gunningscriteria (eerste in ranking), 25% van het volume aan de inschrijver die de op één na hoogste score haalt (tweede in ranking) en 25% van het volume aan de derde in ranking. Voor de overige Deelnemende Entiteiten met name; RZ Tienen, Sint-Trudo, Jessa Ziekenhuis Hasselt zal de afname voor dit perceel gebeuren bij de leveranciers die als eerste twee in de ranking staan. 60% van het volume wordt afgenomen bij de inschrijver met die de hoogste totale score haalt op de gunningscriteria (eerste in ranking), 40% van het volume bij de inschrijver die de op één na hoogste score haalt (tweede in ranking).
