Je helpt hen hun zelfredzaamheid te vergroten en stappen te zetten richting participatie, opleiding of werk. Je werkt vanuit het principe van sociale inclusie: de inwoner staat centraal en behoudt zoveel mogelijk eigen regie. Met jouw coachende houding stimuleer je vertrouwen, motivatie en eigenaarschap. Wat ga je doen? Je voert gesprekken en stelt samen met de inwoner een realistisch plan van aanpak op. Je signaleert knelpunten op verschillende leefgebieden en schakelt waar nodig met ketenpartners. Je biedt maatwerk: iedere aanpak sluit aan op de mogelijkheden en behoeften van de inwoner. Je bewaakt de voortgang van je caseload en legt deze zorgvuldig vast in het cli?ntvolgsysteem. Je werkt intensief samen met collega?s en externe partijen zoals werkgevers, zorgaanbieders en onderwijsinstellingen.
