MSH is exploring potential options for the delivery of Residential TCP services to support eligible older people following discharge from hospital.
Residential Transition Care Program provides short term, restorative care designed to optimise functional recovery, promote independence, and support timely transition to the community or longer term care where required. MSH is seeking to understand whether there is market capacity and capability to support delivery of these services at a scale of approximately 10-20 residential beds, in alignment with the requirements of the Aged Care Act 2024 and the Aged Care Rules 2025.
As part of this exploration, MSH is interested in understanding whether residential providers may be able to support the delivery of Transition Care services as Associated Providers, while MSH retains its role as the Registered Provider, including overall program governance, clinical oversight, and statutory accountability.
