Expansion, reconstruction and modernization of hospital building no. 5A for the needs of the Psychiatric Department for Children and Youth

The subject of the procedure is the award of a contract for the execution of the investment task entitled: "Expansion, reconstruction and modernization of hospital building no. 5A for the needs of the Psychiatric Department for Children and Youth", which will be carried out in the "design and build" system. 2. Within the scope of the contract, the Contractor: 1) Shall prepare complete multi-branch design documentation, transfer copyrights to the design documentation to the Contracting Authority and provide author's supervision on the terms described in the draft contract – Annex No. 4 to the SWZ; 2) Shall obtain in the name and for the benefit of the Contracting Authority all necessary agreements, opinions, permits and decisions required by law, including in particular a decision approving the construction design and granting a building permit with a final and enforceable clause, in order to commence and carry out construction works by the Contractor; 3) Shall prepare complete documentation for the interior design and furnishing of hospital building no. 5A, transfer copyrights to the interior design and furnishing documentation to the Contracting Authority and ensure author's supervision; 4) Shall perform construction works based on the design documentation and obtain in the name and for the benefit of the Contracting Authority a final and unconditional decision on the building permit for use of hospital building no. 5A. 3. The Contractor shall carry out the subject of the contract referred to in point 2 above in accordance with the requirements of the Contracting Authority specified in detail in the draft contract – Annex No. 4 to the SWZ. 4. The Contractor shall carry out the subject of the contract in accordance with the requirements of the Contracting Authority specified in this public procurement documentation, including in accordance with the Functional-Utility Program entitled: "Expansion, reconstruction and modernization of hospital building no. 5a to adapt the facility to the needs of the inpatient ward for the treatment of neurotic disorders for children and adolescents", authored by KKAD Sp. z o.o. based in Krakow; the opinion of the Municipal Conservator of Monuments of June 05, 2018 – letter no. KZ03.4120.6.174.2018.MT and decision no. AU2/6733/257/2018 of July 18, 2018 on the determination of the location of public investment. 5. The Contractor shall perform the subject of the contract in accordance with applicable law, as well as with the best knowledge and experience, with the utmost diligence and in accordance with the principles of technical knowledge and regulations and standards in force on the date of final acceptance, which also means that if the scope of construction works lacks detailed descriptions of their execution technology or other activities necessary for the correct execution of construction works are not indicated, and the given technology or construction practice suggests their execution, the Contractor should perform such works.
