Description
The provider will work with stroke survivors to understand their support needs across multiple health and social care services. The contract aims to provide a high quality, non-discriminatory service to:
Support stroke survivors to maintain their own health and wellbeing and live as independently as possible in their own homes and community as appropriate.
Provide timely and appropriate information, emotional support, practical advice and informal advocacy support for stroke survivors, their families and informal carers.
Provide stroke survivors and their families and informal carers with a better understanding of the effects of stroke;
The Stroke Recovery Service (hereinafter referred to as 'the Service') is provided by Stockton-on-Tees Borough Council and supports people who have survived a stroke to receive support, information and social interaction following their discharge from hospital. It is an element of the Acute Stroke Pathway in Stockton
on-Tees, which also includes the Community Stroke Team (CST) commissioned by the North East and North Cumbria Integrated Care Board (ICB) as part of the Community Integrated Assessment Team (CIAT). The CST provides rehabilitation services, including physiotherapy, occupational therapy, speech and language therapy, and makes referrals for specialist therapies where appropriate. The Service will be available to all stroke survivors who live in the Borough of Stockton-on-Tees and who are registered with a GP in Stockton-on-Tees.
To aid recovery and reduce the risk of stroke recurring, the Service is required
to:
• Support stroke survivors in identifying and working towards their own
needs, wants and goals with specialist input and guidance as required.
• Maintain regular contact with the stroke survivor as appropriate, including
but not limited to home visits, face-to-face meetings, telephone calls, virtual
meetings and sending out information.
• Conduct regular reviews of each stroke survivor's needs.
• Guide people using the service through lifestyle changes based on
identified risk factors and provide stroke survivors and their families and/or
informal carers with practical support and advice.
• Support stroke survivors to regain confidence, re-engage in community
activities that are relevant to them as individuals, and aid the reduction of
social isolation.
• Make appropriate onward referrals where additional needs are identified
and with the consent of the person using the service.
Stockton On Tees Borough Council intends to award this contract to the existing provider following the Provider Selection Regime Option C. The contract value will be £38,000 per year.
