Provision of home care services for persons requiring assistance from the city of Wałbrzych and provision of specialized care services

The subject of the order is the provision of home care services for persons requiring assistance from the city of Wałbrzych and the provision of specialized care services, referred to in the Act of March 12, 2004 on social assistance (Journal of Laws of 2024, item 1283), hereinafter referred to as "services", (excluding specialized services for people with mental disorders) for wards of the Municipal Social Welfare Center in Wałbrzych. These persons, due to age, illness or other reasons, as well as due to their health condition, which causes difficulties in daily functioning, require the help and care of other people. Specialized care services are tailored to the specific needs resulting from the type of illness or disability, provided by people with specialized professional training. I. Description of the Subject of the Order regarding the provision of home care services for persons requiring assistance from the city of Wałbrzych. 1. The subject of the order is the provision of care services, in accordance with art. 50 sec. 3 of the Act of March 12, 2004 on social assistance (Journal of Laws of 2024, item 1283) for persons who are wards of the contracting authority, i.e. persons from the city of Wałbrzych. 1) the number of people covered by the services - approx. 210 people. 2) services should be provided from Monday to Saturday and on public holidays. 3) on average per month, the number of hours of services provided will be approximately 10,200 clock hours, including approx. 1,000 hours provided on public holidays. The number of service hours over the entire contract period is 280,500 hours. 4) services granted in the amount of at least two hours a day may be provided in two parts, care services may also be provided in the afternoon hours as needed. 5) if one caregiver provides services to two or more wards in one day, the Contractor must take into account the time necessary for the caregiver(s) to travel between environments. It is not permissible to include this time in the hours of service provided to the wards. 6) in particularly justified cases, services may be ordered by the contracting authority by phone, and their confirmation will take place by submitting the order within 7 days from the date of notification. In the absence of the possibility of determining the income situation and thus issuing the order, immediately after completing the required documentation. The ward must be covered by services no later than 3 hours from the moment the contracting authority reports an emergency, when care services must be provided immediately. Approximately 10 such cases occur per year. 7) The Contractor should provide the possibility of telephone contact during the contracting authority's office hours, i.e. 7:30 AM - 3:30 PM, as well as after these hours, in order to report emergencies for immediate service assistance. 8) The Contractor is obliged to provide the contracting authority with written information along with the invoice containing the data of the persons providing the service, the date of its provision to each ward (along with the ward's data and the actual number of service hours), the Contractor is also obliged to inform the contracting authority about changes in the event of changes in the above-mentioned data.
