Provision of sports and recreational services for employees, former employees of University Hospital No. 1 named after dr. A. Jurasza in Bydgoszcz who have retired or are on disability pension and have simultaneously terminated their employment with the Hospital, and their eligible family members.

1. The subject of the order is the "Provision of sports and recreational services for employees, former employees of University Hospital No. 1 named after dr. A. Jurasza in Bydgoszcz who have retired or are on disability pension and have simultaneously terminated their employment with the Hospital, and their eligible family members.", in the form of entitlement to free and unlimited access to partner facilities (hereinafter referred to as sports facilities) and sports and recreational activities of a diverse nature for a group of approximately 1250 users (with an employment of about 2200 people) for an estimated period of 48 months. The service will be provided under a monthly subscription, in which users should be provided with access to sports facilities and the opportunity to use the sports and recreational services offered there. User – an employee of University Hospital No. 1 named after dr. A. Jurasza in Bydgoszcz, a former employee of University Hospital No. 1 named after dr. A. Jurasza in Bydgoszcz who has retired or is on disability pension and has simultaneously terminated their employment with the Hospital, and the employee's family members. Family members of the employee are considered to be: Dependent children, adopted children, and children taken in for upbringing within a foster family, up to the age of 18, but not longer than up to the age of 25 if they continue their education in full-time studies (confirmed by a school certificate), as well as regardless of age, children with a significant or moderate degree of disability, and spouses within the meaning of the Act of March 4, 1994, on the Company Social Benefits Fund. The provision of the subject of the order should be carried out based on: - presentation of a personal card in physical form or in electronic form via an application provided by the Contractor, or another method excluding verification using biometric data. In the case of providing an application or another method, it must allow the use of the sports and recreational program without current internet access; - confirmation of identity using an appropriate document. 2. Contractors who meet the conditions regarding: technical or professional capacity may apply for the order: The contractor will meet the condition of participation in the proceedings regarding technical or professional capacity if they demonstrate that: In the period of the last 3 years before the deadline for submitting offers, and if the period of business activity is shorter – during this period, they have properly performed at least 2 services consisting of the performance/execution of a service corresponding in type and scope to the service constituting the subject of the order, with a value of not less than PLN 300,000.00 (in words: three hundred thousand zlotys 00/100) each. A service corresponding in type to the service constituting the subject of the order should be understood as a service consisting of providing access to at least 50 sports facilities in Bydgoszcz and to at least 3300 sports facilities throughout the country (including facilities located in Bydgoszcz).
