Description

This funding supports Winter in the West system for the following elements:
• Frailty Same Day Emergency Care (FSDEC) at weekends. £150,000. This would provide cover to match opening hours of SDEC at weekends. This will support flow through ED, admission avoidance through early intervention and treatment, better outcomes and better patient experience. It will also support QEH 4 hour and 12-hour improvement trajectories. Currently c12-15 patients a day are seen through FSDEC.
• ARI bookable capacity in GP Front Door (GPFD). £125,000 to extended GPFD service and the creation of directly bookable slots. This would provide additional evening cover, as well as creating c20-25 bookable slots for GP direct access throughout the day.
• Aries transport to support discharge. £112,000, this would provide cover 7 days per week for 10 hours per day. This is expected to reduce discharges that fail for transport reasons by circa 20-25 per week. Therefore, reducing length of staff for these patients by at least one extra night or sometimes more.
The duration of this modification is 15th December 2025 - 31st March 2026 and the value is 387,000.00.
There is no change in contract dates or term which remains 1 April 2022 - 31 March 2030, inclusive of 3 year option to extend.
This modification relates to a Norfolk and Waveney ICB contract. As of 1 April 2026, this authority has been dissolved and replaced by NHS Norfolk and Suffolk ICB.
