DETAILED DESCRIPTION OF THE SUBJECT OF THE ORDER: The order includes: 1) service of conducting individual pedagogical therapy sessions for project participants, persons in foster care, 2) conducting functional diagnostics and determining the needs of supported persons, 3) developing and implementing an individual session plan, 4) conducting activities to improve cognitive functions (attention, memory, visual and auditory perception, visual-auditory coordination) and developing reading, writing, counting skills and learning techniques 5) strengthening motivation, independence and social competences, 6) maintaining documentation of the implemented subject of the order, 7) the total scope of the service is 430 (clock) hours, an average of 13 hours/month of the subject of the order for project participants from the date of signing the agreement until August 31, 2029, 8) the services will cover persons indicated by the Ordering Party, in the number of hours of sessions of the subject of the order determined by the Ordering Party, 9) it is assumed that during the implementation of the order, the final number of project participants, and thus the number of hours of support, may change depending on recruitment, resignations and acceptance of subsequent persons into the project and the determination of individualized needs of project participants – the maximum decrease/increase may change by up to 20% of the value, 10) the total involvement in the implementation of all projects financed by EU funds and activities financed from other sources, including the beneficiary's own funds and other entities (regardless of the form of involvement), of the person directly implementing the subject of the order, cannot exceed 276 hours per month. Conditions for amending the agreement: 1. No changes to the provisions of the agreement are allowed in relation to the content of the offer on the basis of which the Contractor was selected, subject to the following provisions. 2. In the event of random events and special circumstances that could not have been foreseen at the time of concluding the agreement, it is possible to introduce changes to the concluded agreement in the scope of: a) changes in the name, designation, registered office, bank account number of the Ordering Party or Contractor, b) postponement of the deadline for the performance of the subject of the order, c) changes in persons dedicated to the performance of the subject of the agreement in case of impossibility of performing their assigned tasks for objective and independent reasons from the Contractor, e.g. illness confirmed by a sick leave certificate or maternity leave. The new person must be approved by the Ordering Party, with the reservation that the person will have qualifications and experience not lower than those specified in the announcement and not lower than those offered in the offer (does not apply to the Contractor – a natural person not running a business activity), d) the final number of project participants, and thus the number of persons covered by the support of the subject of the order and the number of units or hours of support may change depending on recruitment, resignations and acceptance of subsequent persons into the project and the determination of individualized needs of project participants – the maximum decrease/increase may change by up to 20% of the stated values, e) the number of forms of support indicated in the tender request results from the project co-financing provisions and may change at the project implementation stage, with the reservation that the unit price will be maintained. 3. Changes to the agreement may be initiated by either Party, in writing, with appropriate justification for the requested change. 4. All changes to the provisions of the agreement require a written form under pain of nullity.
