A detailed description of the subject of the order referred to in point 3.1 is included in Annex No. 10 a and b to the SWZ – Description of the Subject of the Order, Annex No. 4 to the SWZ – Offer Form, Annex No. 8 to the SWZ – Draft Agreement. 6.3. If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in paragraph 6.2, points 1, 2, 3 of this chapter, it shall submit: a. information from the National Criminal Register, referred to in art. 108, paragraph 1, points 1, 2 and 4 – submit information from the relevant register, such as the court register, or in the absence of such a register, another equivalent document issued by the competent court or administrative authority of the country where the contractor has its registered office or place of residence or where the person concerned resides or the document, b. instead of a certificate from the competent tax office confirming that the contractor is not in arrears with taxes and fees, within the scope of art. 109, paragraph 1, point 1 of the Act, instead of a certificate or other document from the competent local unit of the Social Insurance Institution or the competent local branch of the Farmers' Social Insurance Fund within the scope of art. 109, paragraph 1, point 1 of the Act, instead of a copy or information from the National Court Register or the relevant register or from the Central Register and Information on Economic Activity, within the scope of art. 109, paragraph 1, point 4 – submit a document or documents issued in the country where the contractor has its registered office or place of residence, confirming respectively that: 1) it has not violated obligations regarding the payment of taxes, fees or social or health insurance contributions, 2) it has not been subject to liquidation proceedings, bankruptcy proceedings have not been declared, its assets are not managed by a liquidator or a court, it has not entered into an arrangement with creditors, its economic activity has not been suspended, nor is it in any similar situation resulting from a similar procedure provided for in the regulations of the place where the procedure was initiated. 6.4. If in the country where the contractor has its registered office or place of residence or where the person concerned resides, documents referred to in point 6.3, point a. are not issued, or if these documents do not relate to all cases referred to in art. 108, paragraph 1, points 1, 2 and 4, art. 109, paragraph 1, point 1, 2 letter a and b, and point 3 of the Act, they shall be replaced in whole or in part by a document containing the contractor's statement, indicating the person or persons authorized to represent it, or a statement by the person concerned, made under oath, or, if in the country where the contractor has its registered office or place of residence or where the person concerned resides, there are no provisions for statements under oath, made before a court or administrative authority, notary, professional or economic self-government body, competent for the contractor's registered office or place of residence or the place of residence of the person concerned. 6.5.
