Op deze opdracht kunnen geen zelfstandigen worden aangeboden. Het gaat om alle voorkomende werkzaamheden binnen de Wmo, van huishoudelijke hulp tot begeleiding en complexe woningaanpassingen. Als WMO consulent verhelder je de vraag van de burger en geef je informatie en advies over de Wet Maatschappelijke Ondersteuning en collectieve voorzieningen; zo nodig verwijs je door. Tijdens het onderzoek en het huisbezoek ben je in staat om te kunnen beoordelen welke maatwerkvoorziening het beste past op de situatie van de burger. Je rapporteert en beslist zelfstandig op aanvragen. Je hebt een integrale kijk op vragen op het gebied van beschermd wonen en je helpt om de burger te versterken in zijn of haar eigen kracht en zelfredzaamheid. Verder: ? Breng je de sociale omgeving in kaart en bespreek je met de inwoner hoe deze in te zetten. ? Rapporteer en beslis je zelfstandig op aanvragen en ben je in staat om de bijbehorende administratie te doen. Na gunning van de opdracht dient de gegunde partij een recente VOG aan te bieden
