Execution of complete design and cost estimation documentation for the public utility building Grochowska 259 A

1. The subject of the order is: the execution of complete design and cost estimation documentation for the public utility building Grochowska 259 A, in the form of: execution of multi-branch design and cost estimation documentation for the reconstruction (revitalization) with the addition of 1 (one) floor, modification of the functional layout, reconstruction of staircases with the addition of elevators, thermal modernization, and development of the adjacent area, for the utility building at ul. Grochowska 259A in Warsaw. 2. Currently, the building houses care and day support facilities (including, among others, Community Support Centers for people with intellectual disabilities and chronic mental disorders). The facility is intended for the day care of beneficiaries. Due to the specific nature of the activity and the caring and therapeutic character of the building, the planned modernization and superstructure must absolutely take into account the highest universal design standards. The modernized facility must be rigorously adapted to the needs of people with special needs (including those with combined disabilities), ensuring their full fire safety, acoustic comfort, and appropriate sanitary and hygienic conditions. 3. The scope of the order includes the execution of all studies, analyses, designs, and documents necessary for the preparation of the investment, including obtaining the required agreements, opinions, and administrative decisions, removing detected defects, irregularities, and functional limitations of the facility, as well as adapting it to the new functional use, respecting its historical and architectural values. 4. The documentation must be complete from the perspective of its purpose, i.e., enabling the public procurement procedure for construction works, the execution of construction works, and the achievement of the intended technical, energy, functional, and aesthetic effect, including enabling the Contractor to prepare a complete description of the subject of the order for construction works in accordance with the Public Procurement Law. 5. The detailed scope of the order for Stages I-III is specified in the Draft Agreement, including the description of the subject of the order and its attachments, which is Attachment No. 4 to the SWZ. 6. The scope of the order also includes Stage IV "Designer's participation in the tender procedure on the part of the Contracting Authority", for "Performance of author's supervision" and for "Execution of energy performance certificates for each building", which will be signed no later than 3 days from the commencement of the public procurement procedure for the construction works contractor, which contains detailed requirements for the implementation of STAGE IV, which is Attachment No. 5 to the SWZ. This agreement will constitute 10% of the gross value of the offered gross lump-sum price indicated in the offer form.
