1. Matters for Public Competitive Bidding
(1) Contract Name: Relocation of X-ray Fluoroscopy Unit and Mobile X-ray Unit Contract

(4) Place of Performance:
a. Chiba City Mihama Hospital, X-ray TV Room 3, Radiology Department
b. Chiba City Makuhari Hospital, X-ray TV Room 3, Radiology Department
2. Eligibility for Participation in Competition
Those wishing to participate in the public competitive bidding must meet all of the following requirements.
(1) Persons who have undergone the examination for eligibility to bid for goods in Chiba City for FY2026-2027 and have been recognized as eligible.
(2) Not falling under the provisions of Article 167-4 of the Local Autonomy Act Enforcement Order (Order of the Ministry No. 16 of 1947), and not falling under any of the following:
a. Persons who have not passed two years since being subject to a transaction suspension order by a clearing house.
b. Persons who have issued dishonored bills or dishonored checks within six months prior to the bid date.
c. Persons who have filed for commencement of rehabilitation proceedings under the Corporate Reorganization Act (Act No. 154 of 2002) and for whom a decision to commence rehabilitation proceedings has not been made by the court under the same Act.
d. Persons who have filed for commencement of rehabilitation proceedings under the Civil Rehabilitation Act (Act No. 225 of 1999) and for whom a decision to approve the rehabilitation plan has not been made by the court under the same Act.
e. Persons who are subject to a suspension of bidding eligibility or similar measures under the Chiba City Measures for Suspension of Bidding Eligibility for Goods, etc. (enforced August 1, 1985) during the period from the application deadline for bidding eligibility to the bid date.
f. Persons who are in violation of the City Planning Act (Act No. 100 of 1968) within Chiba City.

4. Distribution and Submission of Application for Confirmation of Bidding Eligibility
Those wishing to participate in the public competitive bidding must apply for participation in the competition.
(1) Distribution of Application Forms, etc.: Download from the link for this project in the "List of Procurement Information" under "Goods" on the Chiba City "Bidding Information" portal page.

(2) Submission Location, etc.: Submit by mail or in person to the Contract Administration Section by September 15, FY2024 (Tuesday), from September 11, FY2024 (Friday). For in-person submission, between 9:30 AM and 4:30 PM, excluding Sundays, Saturdays, and holidays.
(3) Confirmation Notification: A notification of the results of the bidding eligibility confirmation will be sent to the applicant by September 18, FY2024.

6. Bidding Procedures, etc.
(1) Date and Time of Bidding and Opening of Bids: 10:00 AM on September 28, FY2024 (Monday) (For mail-in bids, by 5:00 PM on September 25, FY2024).
(2) Bidding Method: Lump sum bid.
Submit the bid form by registered mail or in person to the Contract Administration Section.
(3) Bid Bond: Exempt (However, if it falls under the provisions of Article 8, Paragraph 2 of the Chiba City Contract Regulations, which are applied by analogy according to the Chiba City Hospital Bureau Contract Regulations (Chiba City Hospital Bureau Regulations No. 25 of 2011), the amount equivalent to the exempted bid bond will be collected as a penalty).
(4) Method of Deciding the Successful Bidder: The bidder who submitted a valid bid at the lowest price within the limit of the estimated price prepared in accordance with the provisions of Article 10 of the Chiba City Contract Regulations, as applied by analogy by the Chiba City Hospital Bureau Contract Regulations, will be the successful bidder. If the bid amount is less than the minimum reserve price, the bid will be disqualified. If there are two or more bidders who submitted the same lowest valid bid, the successful bidder will be determined by lottery.
(5) Invalid Bids: Bids that fall under the provisions of Article 16 of the Chiba City Contract Regulations, as applied by analogy by the Chiba City Hospital Bureau Contract Regulations.
7. Other
(1) Security Deposit: Required (However, it will be exempted if it falls under Article 29 of the Chiba City Contract Regulations, as applied by analogy by the Chiba City Hospital Bureau Contract Regulations).
(2) Requirement for Contract Document: Required.
(3) Language and Currency to be Used in Contract Procedures: Japanese language and Japanese currency only.
