1. Szczegółowy opis przedmiotu zamówienia znajduje się w załączniku nr 4 do SWZ - OPZ 2. Warunki realizacji zamówienia określono w Projektowanych postanowieniach umowy stanowiących załącznik nr 2a do SWZ. 3. Zamawiający przewiduje możliwość skorzystania z prawa opcji. 4. Rodzaj (asortyment), oraz ilości przedmiotu zamówienia, określono załączniku nr 3 do SWZ – Formularz cenowy. 5. Zaoferowany przedmiot zamówienia musi spełniać wymagania określone 3 do SWZ – Formularz cenowy, jak również w załączniku nr 2a do SWZ – Projektowane postanowienia umowy oraz w OPZ - załącznik nr 4 do SWZ. 6. Warunki realizacji zamówienia określono w Projektowanych postanowieniach umowy stanowiących załącznik nr 2a do SWZ. 7. W celu potwierdzenia, że oferowany do dostawy asortyment spełnia określone przez zamawiającego wymagania, do oferty należy załączyć przedmiotowe środki dowodowe zgodnie z rozdziałem IV SWZ (w tym złożyć z ofertą Kartę katalogową produktu oraz wypełnić i złożyć OŚWIADCZENIE WYKONAWCY stanowiący załącznik nr 8 do SWZ).
