task 1 Water for injection/infusion and preparation of medicines - standing bags with two ports

1. The subject of the order is the supply of infusion fluids for the needs of USK-2 PUM Clinics. The description of the subject of the order and the estimated quantities of the products are included in the unit price form (attachment 2 to the SWZ, hereinafter also referred to as FCJ). 2. The Ordering Party requires the offer of: a) medical devices – within the meaning of the Act of April 7, 2022 on medical devices (Journal of Laws 2024.1620 as amended, hereinafter: "UoWM"), introduced into circulation and use in the territory of the Republic of Poland in accordance with applicable regulations - in the scope of task no. 2, items 1 and 2; task no. 10, item 1, and task no. 11, items 4, 5, 6, 7; b) medicinal products in accordance with the Act of September 6, 2001, Pharmaceutical Law (Journal of Laws 2001.126.1381 as amended), introduced into circulation and use in the territory of the Republic of Poland in accordance with applicable regulations, in the scope of the remaining tasks and items. 3. The Ordering Party requires that the shelf life of all offered products is a minimum of 12 months, counting from the date of delivery to the Ordering Party's registered office. 4. The quantities of the assortment constituting the subject of the order, specified in the annex to the contract (TASKS NO. 1-15), are estimated values, used for correct calculation of the offer price, comparison of offers and selection of the most advantageous offer. The Ordering Party has the right to order smaller quantities, but not less than 60% of the value of the contract/task. The quantity of the ordered assortment, within the scope of contract execution, may be reduced or increased, within individual assortment items, depending on the actual needs of the Ordering Party, with the reservation that the value of the offer and the Contract of the selected Contractor will not be exceeded. The Ordering Party has the right not to fulfill the full quantity and assortment of the contract. In such a situation, the Contractor will not be entitled to any claims. The Ordering Party has the right not to fulfill the full subject of the contract. 5. The order consists of 15 tasks, the detailed description of which is included in attachment 2 FCJ. 6. The Ordering Party does not allow for variant offers. 7. The Ordering Party requires the execution of the subject of the order for a period of 24 months, counted from the date of the contract conclusion.
